Job overview Gloucestershire Hospitals NHS Foundation Trust wishes to appoint two consultants in Upper GI Surgery to develop the Emergency General Surgery service as part of a phased expansion of the service. This is an exciting new hybrid post to support established pancreatobiliary, oesophagogastric and colorectal teams and expand and develop emergency surgical care as part of a larger general surgical team. We are looking for two keen, dynamic Consultants willing to integrate with the existing subspecialty teams of 18 surgeons who provide emergency general surgery and specialist on call for the patients of Gloucestershire. It is anticipated that the successful candidates will work with the existing teams to share the inpatient workload, provide rapid access to a senior decision maker and increase the efficiency of CEPOD lists. They will be expected to further develop ambulatory pathways and establish hot clinics to help reduce attendances on SAU. Initially they will contribute to the weekend upper GI on call rota but this is likely to change with the appointment of further surgeons into these roles. The post holders will also have an elective component to their job plan to help meet waiting list targets for general surgical procedures, particularly cholecystectomy.

Main duties of the job

• Clinical Care: Deliver high-quality patient care, including assessment, treatment and ward rounds, and provide clinical support to junior medical and nursing staff. Participate in the agreed on-call and out-of-hours rota.
• Clinical Leadership: Provide senior clinical support and work collaboratively with the multidisciplinary team to ensure safe, effective and coordinated care.
• Education and Training: Contribute to undergraduate and postgraduate teaching and support the development of junior medical staff. Act as Educational Supervisor where agreed.
• Management: Participate in departmental and Trust meetings and contribute to service development and improvement.
• Clinical Governance: Promote evidence-based practice, follow NICE and professional guidance, and participate in clinical audit, risk management, quality improvement and management of incidents and complaints.
• Professional Development: Maintain CME/CPD and participate in annual job planning and appraisal.
• Research: Support and participate in clinical research where appropriate.
• Professional Responsibilities: Work in accordance with Trust policies and procedures and contribute effectively to team-based working

Detailed job description and main responsibilities 1. Clinical Responsibilities/Key Responsibilities

• To share responsibility for the delivery of high-quality care to the patients presenting to the department.
• To examine and treat patients on the shop floor of the Department.
• To support the clinical decision making of junior medical and nursing staff.
• To provide on call support out of hours. To attend the Department out of hours as per agreed protocols. To remain contactable and available when on call.
• To carry out ward rounds.

2. Professional Development, Education and Training

• To provide conditions for improved training opportunities in line with national and local recommendations.
• To participate in the undergraduate teaching programme.
• To assist and participate with the development of postgraduate training for F1s, F2s, SpRs, STs and other staff as appropriate.
• Where agreed, to act as Educational Supervisor to junior and middle grade medical staff, being responsible for their appraisal and personal development planning. This would be agreed at initial job-plan review (6-12 months) and reflected in SPA allocation.

3. Management and Leadership

• To provide medical information for the development of systems appropriate for Trust needs.
• To participate in departmental consultant and senior staff meetings whilst remaining within the framework of the agreed strategy of the Trust and the Department.
• To attend other departmental, Divisional and Trust meetings as necessary.
• To attend regional and national meetings as necessary.
• To undertake all work in accordance with Trust procedures and operating policies.

4. Research and Development

• The post-holder will be encouraged to be active in clinical research.

5. Communications and Working Relationships The Upper GI team has an inclusive approach to team working and it is expected that the appointee would embrace this approach and would also be expected:

6. Clinical Governance

• To undertake all work in accordance with Gloucestershire Hospitals NHS Foundation Trust procedures and operating policies.
• To attend accredited conferences and meetings to update personal level of clinical practice, teaching and management skills in line with CME/CPD requirements.
• To participate in an annual Job Planning Review process.
• To participate in the Trust's annual Appraisal process and to respond to any identified development needs.
• To adhere to the Trust's Adverse Clinical Incident Policy.

7. Other Audit (as agreed with Service Line Director)

• To promote evidence based practice. To respond appropriately to NICE guidance. To change own and others clinical practice in response to new clinical developments.
• To undertake and supervise relevant clinical audit topics.
• To be involved in risk management and quality assurance.
• To remain up to date and engaged in CME/CPD as per RCS guidelines.
• To respond appropriately to complaints and critical incidents.
• The post holder will be expected to take part in developing clinical audit in accordance with agreed Directorate and Corporate Clinical Governance programmes. These build on the considerable progress already made by the Trust in clinical audit and continuous quality improvement.
