Job Description
Primary Integrated Community Services are supporting Bassetlaw Hospice with the recruitment for the following vacancy.
Thepost holder will have a service wide remit to support services to deliver excellentpalliative and end of life care throughout In-Patient Unit (IPU) CommunityServices (CNS Team) and the Day services/ Wellbeing services. This post holderwill also be expected to deliver teaching and supervision to the AdvancedClinical Practitioners (ACPs) and Clinical Nurse Specialist (CNS)
TheService operates throughout Bassetlaw, providing palliative and end of lifecare and services to all patients registered with a Bassetlaw GP, withconsideration to out of area patients on an individual basis.
Thispost holder will report directly into the Director of Clinical Services, andaccess supervision via the GMC. The post holder will create networks with otherhospice and palliative care colleagues to strengthen the alliances andknowledge base for the benefit of the patients and those
Were hiring this role as an employed member of staff, an a fixed term contract for two years. You'll be part of the team, on payroll, with all the support and benefits that come with it. Not on a self-employed basis.
This role is offered on a 4-day per week basis however this can be negotiable.
Main duties of the job KEY TASKS
Bassetlaw Hospice, Retford, North Nottinghamshire has 6 inpatient beds supported clinically by established Advance Clinical Practitioner roles. It also provides clinical nurse specialist support to the community as well as day therapy and outpatient clinics. There are excellent links with local secondary care, primary care and ICB colleagues as well as aspects of palliative care delivered via the Nottinghamshire Healthcare Trust community Services.

Job responsibilities Duties of thePost
As Consultant in PalliativeMedicine you will provide the following:
Theinitial job plan will be agreed flexibly with the successful candidate and willbe subject to negotiation and review. Three months after commencement, the jobplan will be reviewed and agreed between the appointee and the Director ofClinical Services. This will be a prospective agreement that sets out theconsultants duties, responsibilities and objectives for the coming year, andwill be reviewed annually. It should cover all aspects of a consultantsprofessional practice including clinical work, teaching, research, educationand managerial responsibilities. In addition, it should include personalobjectives, including details of their link to wider service objectives, anddetails of the support required by the consultant to fulfil the job plan andthe objectives.
On call availability
Out ofhours activity is currently delivered by Derbyshire Health United (DHU)practitioners who then access on call consultants via South Yorkshire orNottinghamshire dependant on area. This will be reviewed and developed in linewith service need. It is possible that an on-call requirement may becomenecessary in the future.
Therole will require oversight of clinical education for Palliative and End ofLife Care across Bassetlaw Hospice and to colleagues in partner organisations,to ensure services are educated and fit for purpose. The appointee is expectedto participate in teaching in collaboration with the education lead.
Thepost holder will be encouraged to contribute to research activities at thehospice and contribute to audit and quality improvement projects.
Theappointee is expected to take part in continuing professional medicaleducation; study leave is provided and funding is available on application.
AppraisalRevalidation and Job Planning
Consultantsare expected to undertake the annual Job Planning process which is carried outin collaboration with the Director of Clinical Services. The Consultant willbring relevant data on workload in the previous year to the meeting.
Thereis a new starter Trust induction programme for all staff and a New ConsultantInduction Programme Prior to commencing post, the Consultant will be informedof the corporate and local induction processes and programme. It is expected that the Consultant willparticipate fully in this and keep up to date with mandatory and statutorytraining in line with Trust requirements.
Newly appointedConsultants will be encouraged to seek a mentor to support their development intheir first year of appointment.

- Deliver consultant level day-to-daymedical care to hospice in-patients and patients accessing other SpecialistPalliative Care services.
- Provide clinical leadership, educationand development to the Advanced Clinical Practitioners
- Provide leadership and participate as amember at the weekly in-patient multidisciplinary team meeting.
- Responsible for review of current out ofhours cover and implementation/ development of the robust out of hours cover.
- Cover for colleagues annual leave andother authorised absences. This post is alongside an in house GP working 2 daysper week.
- Any responsibility which relates to aspecial interest.
- Educational and clinical supervision oftrainee doctors, advanced clinical practitioners and supervision of SpecialtyDoctors. We currently do not have trainee doctors but this will be a futurerequirement.
- Training and supervision of NurseIndependent Prescribers.
- To participate in education and trainingjunior doctors, medical students, hospice staff and volunteers.
- Participate in clinical audit, qualityimprovement projects and clinical governance processes.
- Support research activity.
- Comply with Continuing ProfessionalDevelopment (CPD) requirements as stipulated by the Royal Colleges.
- To keep a portfolio of evidence, to havean annual appraisal and write a personal development plan (PDP) in line withGMC revalidation policies and procedures.
- Support and assist in the achievement ofHospice strategy and objectives.

Person Specification Qualifications
- Full registration with the UK General Medical Council with a licence to practise
- MRCP
- Eligible for entry on GMC Specialist Register or within 6 months of receipt of Certificate of Completion of Training (CCT) at time of interview, or Certificate of Eligibility for Specialist Registration (CESR)
- Higher qualifications relevant to Palliative Medicine eg M.D., MSc, Ph.D
- Ability to communicate effectively with patients, relatives, colleagues
- Evidence of ability to work in a team and delegate appropriately
- Good interpersonal skills
- Evidence of good personal organisation, including time management
- Work within a multidisciplinary team, both as a member and as a leader
- Enquiring and critical approach to work
- Commitment to Continuing Professional Development
- Willingness to undertake additional professional responsibilities at local, regional or national level
- IT Skills
- Able to travel within the catchment area
- Evidence of attendance on an Advanced Communication Skills Course
