This role is ideal for nurses with a background in ICU, HDU, or complex care who are ready to embrace more autonomy, flexibility, and a genuine work life balance - all while continuing to make a real impact in people's lives.
This is a full time, permanent role working as a Nurse Case Manager (complex care), in the Merseyside and Cheshire region. The role is to work Monday to Friday, 9am - 5pm, with no nights, weekends, or bank holiday working. The salary is £43,000 per annum. You must be a driver with car for this community role.
What You'll DoAs a Community Registered Nurse, you'll play a key role in supporting and overseeing high-quality care for individuals with complex health needs, including those with:
- Spinal or neurological injuries
- Acquired brain injuries or trauma
- Epilepsy, learning disabilities, or genetic disorders
Your responsibilities will include:
- Assessing, reviewing, and maintaining bespoke care packages
- Providing hands on clinical training and supervision to care staff
- Offering expert guidance and support to clients and their families
- Collaborating with health and social care partners (ICBs, hospitals, case managers, and other professionals)
- Participating in a paid on call rota on a rotational basis
