We're looking for a compassionate and dedicated Community Rehabilitation Assistant to join our dynamic team in Ballyhea, Co. Cork, where person centred care is at the heart of everything we do.
About the RoleAs a Community Rehabilitation Assistant, you will play a key role in supporting adults living with an acquired brain injury to rebuild independence, confidence, and meaningful participation in their own communities. Working as part of an interdisciplinary, person centred team, you will deliver one to one rehabilitation supports in people's homes and local environments, helping them work toward goals that matter to them - whether that's daily living skills, social connection, cognitive strategies, or community engagement. This role is dynamic and community based, offering variety, meaningful relationships, and the opportunity to make a tangible difference in someone's everyday life.
The contracts available are:
- Specified Purpose, Part Time, 19.5 hours
- Relief
The salary range is €30961-€44534 - DOE, pro rated based on hours worked.
What You'll Be Doing- Build respectful, trusting relationships and offer consistent support
- Help create and guide personalised rehabilitation plans
- Encourage independence through daily living skills, personal care, and goal focused activities
- Identify additional support needs for the person served
- Encourage family, personal and neighbourhood relationships and interactions
- Feed back to the clinical and management team throughout the rehabilitation journey
- Play a key role in case coordination for supporting consistency in rehabilitation review meetings
We're seeking a person centred candidate who is compassionate, reliable and genuinely motivated by supporting others to achieve their goals. You will thrive in this role if you are patient, adaptable, and comfortable working independently in a community setting.
You will have strong communication skills with an ability to build trust and support people at their own pace. You will have a problem solving mindset and a commitment to supporting people to make choices, develop skills, and participate fully in their communities.
Must Haves- Relevant Health or Social Care qualification to a QQI Level 5 or higher
- Full driving licence

- Confident using basic IT systems to stay connected and organised
- Experience in Residential or Community settings
- Background in Disability or Illness support
