Clinical Prescribing Pharmacist

We are looking for an experienced, enthusiastic, and highly efficient Clinical Pharmacist to join our fast-paced, forward-thinking team. This is a key clinical role supporting the safe, effective, and evidence-based use of medicines across the practice. The ideal candidate will be confident working independently while also contributing to multidisciplinary care. This evolving role offers significant development opportunities as we expand our PCN team.
Main duties of the jobThe clinical pharmacist will primarily focus on delivering high-quality, patient-centered pharmaceutical care.
- Patient Care & Clinical Assessment: Act as an integral member of the multidisciplinary team, delivering patient-facing and telephone consultations. Clinically assess and manage care for defined patient groups, focusing on long-term conditions. Use expertise to make prescribing and treatment decisions aligned with best practices.
- Medication Reviews & Consultations: Conduct structured medication reviews for high-risk patients and those with long-term conditions. Collaborate with patients to optimize medication regimens, recommend evidence-based changes, and improve safety and cost-effectiveness. Enhance patient and carer understanding of medicines to boost adherence and outcomes. Respond to medicines-related queries from healthcare professionals, patients, and carers, providing expert advice on adverse reactions, interactions, and urgent medication needs. Oversee medicines optimization systems, manage prescription requests, coordinate prescriptions for care homes, troubleshoot system issues, and review hospital discharge summaries for medication reconciliation.
- Clinical Governance & Quality Improvement: Lead or contribute to clinical audits, support NICE guideline adherence, promote safety, and encourage cost-effective prescribing. Provide training and mentorship to staff, fostering continuous professional development.
- Primary Care Network (PCN) Support: Collaborate with the PCN to deliver services, contribute to prescribing targets, and support PCN objectives including structured medication reviews and health in care homes.
- Administration: Summarize patient records accurately, ensure documentation meets standards, support audits, and adhere to governance procedures.
- Training and Development: Take responsibility for own professional development, attend relevant training, and undertake recertification as needed.
- Governance & Confidentiality: Maintain accurate, confidential records, adhere to governance frameworks, and follow NHS and NICE guidelines.
- Health & Safety: Follow infection control and safety procedures, report incidents, and maintain a safe environment.
- Equality & Diversity: Uphold principles of equality and diversity, ensuring services are delivered respectfully and inclusively.
- Other Duties: Undertake additional duties as required, with roles subject to review.
- Master's degree in pharmacy (MPharm) or equivalent
This post is subject to a DBS check in line with the Rehabilitation of Offenders Act (Exceptions Order) 1975.
Depending on experience, salary may be in line with Band 8a.
