Clinical Pharmacist (Primary Care) - Fixed term/ Mat Cover

There has never been a more exciting time to work in primary care on the Isle of Wight, and Newport Health Centre is looking for a Clinical Pharmacist Team to join the team. We are seeking to recruit a positive, caring and self-motivated individual to take on the role of Clinical Pharmacist.

Main duties of the job
  • Working as part of a multi-disciplinary team in a patient facing role to clinically assess and treat patients within your current competence, using your expert knowledge of medicines for specific disease areas
  • Being responsible for the care management of patients with chronic diseases and undertake clinical medication reviews to proactively manage people with complex polypharmacy
  • Providing specialist expertise in the use of medicines whilst helping to address both the public health and social care needs of patients and to help in tackling inequalities
  • Support the leadership team on ensuring person-centred medicines optimisation (including ensuring prescribers in the practice conserve antibiotics in line with local antimicrobial stewardship guidance) and quality improvement is achieved, whilst contributing to the quality and outcomes framework and enhanced services
  • Support patients to take their medications to get the best from them, reduce waste and promote self-care where possible
  • Support the clinical aspects of shared care protocols, clinical research with medicines, liaison with specialist pharmacists, liaison with community pharmacists and anticoagulation
  • Being part of a professional clinical network and have access to appropriate clinical supervision.

In summary, recogniseand work within own competence and professional code of conduct, to:

  • Contribute to clinical governance activities / clinical audit toensure continual improvement of service delivery.
  • Contribute to complaints responses / reviews, significant event,peer review and other shared learning activities.

  • Support and participate in shared learning across the practice andwider organisation
  • Demonstrate sensitive communication styles to ensure patients arefully informed and consent to treatment

Ensure you are competent and appropriately trained to carry outyour areas of responsibility, including keeping up to date with practice policies and clinical standards

Prioritise own and other's workload and ensure effectivetime-management strategies are embedded within the culture of the team

Use relevant IT systems to support role including SystmOne, TeamNet, Word, Excel, Access, and other systems where necessary

Assist in promoting and maintaining your ownand others' health, safety and security as defined in the Practice Health &Safety Policy and the practice infection control policy and publishedprocedures.

Ensure confidentiality is adhered to at all times

Be flexible in undertaking any other duties asdeemed appropriate to the role

Person Specification Skills
  • Excellent communication and interpersonal skills including emotional intelligence to in order to deal effectively with complex and sensitive patient information, distressed or aggressive patients, or those with an impaired level of understanding.
  • Ability to liaise and communicate effectively with all members of the multi-disciplinary team and other services.
  • Ability to perform holistic assessment, critically analyse complex patient information or situations
  • Ability to critically analyse patient information or situations and make clinical judgements and decisions as to the best course of action
  • Effective time management (Planning & Organising)
  • Ability to work as a team member and autonomously
  • Good interpersonal skills
  • Skilled in clinical history taking
  • Excellent attention to detail
Qualifications
  • Educated to degree/masters level in Pharmacy (Or equivalent)
  • Registered with the GPHC
  • Faculty membership of the Royal Pharmaceutical Society
  • Independent prescribing or working towards/intent of gaining an independent prescribing qualification.
Experience
  • In depth therapeutic and clinical knowledge and understanding of the principles of evidence-based healthcare.
  • Experience of multi-disciplinary working
  • Clear understanding of professional responsibility and accountability
  • Knowledge of current clinical evidence based practice.
  • Experience of delivering patient care in a primary care, urgent or community setting, or willing to gain experience
