As part of our expandingPharmacy Team, you will play a vital role in supporting both patients and practices. The Pharmacy Team is embedded within our broaderAdditional Roles Reimbursement Scheme (ARRS) workforce, which includes a diverse range of professionals working together to enhance patient outcomes. This multidisciplinary approach ensures that our practices are well-supported and that our communities benefit from coordinated, accessible, and compassionate healthcare.
Main duties of the jobTo provide excellent patient care, to improve resilience withinour member practices, and to support practice teams with expert clinicalknowledge of medications. This will be achieved by face-to-face and telephoneappointments, project work around differing long-term conditions as well as CVDprevention, and building positive relationships with key stakeholders.
About usPontefract and Knottingley Primary Care Network (PCN) comprisesfour established GP practices: Ashgrove Medical Centre, Friarwood MedicalCentre, Northgate Medical Centre, and Stuart Road Medical Centre.Collectively, we provide high-quality healthcare services to approximately 49,000registered patientsacross the Pontefract and Knottingley district.
Our PCN is committed to delivering integrated, patient-centeredcare by fostering collaboration across practices and within the widerhealthcare system.

- Independent prescriber with primary care experience
- Skills, knowledge and experience
- Excellent Clinical Knowledge of long-term conditions and common
- Excellent interpersonal, influencing and negotiating skills.
- Excellent written and verbal communication skills
- Demonstrate the ability to communicate complex and sensitive information in an understandable form to a variety of audiences (e.g. patients/ other professionals)
- Competent in prioritisation.
