Job Title: Clinical Pharmacist

Summary: Join our dynamic team at NHS Jobs as a Clinical Pharmacist, providing specialized expertise in medicinal use to address public health and social care needs. Lead person-centered medicines optimization, contribute to Quality and Outcome framework, and engage in locally commissioned services. Conduct structured medication reviews, support patient self-care, and play a pivotal role in shared care protocols. Clinically assess and treat patients using expert knowledge, contribute to effective care transfer processes, and manage patients with chronic diseases. Collaborate on the development and implementation of new services with medicinal components. Ensure compliance with CQC standards and actively contribute to the Medicines Optimisation Scheme. Foster relationships with pharmacy professionals across Primary Care Networks and provide clinical supervision for assigned Pharmacy Technician.
