Clinical Pharmacist - Expressions of Interest We are inviting Expressions of Interest from Clinical Pharmacists who would like to work within our Primary Care Network multidisciplinary team to support medicines optimisation, safer prescribing and long term condition management at Huntcliff Surgery.
This is an opportunity to register your interest in a upcoming ARRS funded Clinical Pharmacist role. Session numbers and responsibilities may be shaped around candidate experience, Independent Prescriber status and service need.
The postholder would work as partof a multidisciplinary team to support repeatprescription reauthorisation, transfer of care medicines management, structured medication reviews and prescribing safety processes.
Candidates will work directly with patients to assess and manage medicines related needs, promote self care and contribute to long term condition management.
The Clinical Pharmacist will work within the PCN Pharmacist Team under appropriate clinical supervision and leadership.
Candidates who have completed the CPPE Primary Care Pharmacy Education Pathway are preferred. Where this has not yet been completed, we will provide structured support and supervision to enable successful completion.
Support toward Independent Prescriber qualification will be provided where appropriate and aligned with ARRS requirements and service need.

Job responsibilities JOBSUMMARY
As a Clinical Pharmacist,the post holder will work as partof a multidisciplinary team to develop and run processes for repeatprescription reauthorisation, management of medicines on transfer of care andsystems for safer prescribing.
They will work directly with patients to assess and treatconditions, manage long-term conditions as well as promote self-care.Theywillbe part of a group of Primary Care Network(PCN)Clinical Pharmacists workingunder the supervision and mentoring of a Clinical Pharmacist Manager.
Candidates who have completed the accredited 18-monthPrimary Care Pharmacy Education Pathway are preferred. Where this has not yet been completed, we will provide structured support and supervision to enable completion. The CPPE pathway does not need to be completed if thepharmacist meets a HEE (Health Education England) exemption such as completionof Clinical Diploma.
PrimaryResponsibilities
Patientfacing Clinical Medication Review:
Undertake clinical medication reviews with patients withinthe practice, care homes and/or domiciliary visit to proactively managepolypharmacy and produce recommendations for senior clinical pharmacist, nursesand/or GP on prescribing and monitoring.
See (where appropriate) patients with single or multiplemedical problems where medicine optimisation is required (e.g. Respiratory,Cardiovascular and Diabetes).
Clinics are done face to face, via telephone, home visitsand in the future, possibly video consultations.
The post holder will undertake clinics for patients ordevelop one to one relationships in order to de-prescribe where necessary oroffer clinical leadership on medicines optimisation and quality improvement.
Review the on-going need for each medicine, a review ofmonitoring needs and an opportunity to support patients with their medicineadherence ensuring they get the best use of their medicines (i.e. medicinesoptimisation). Make appropriate recommendations to GPs for medicineimprovement.
Patientfacing Care Home and Domiciliary Clinical Medication Reviews:
Undertake clinical medication reviews with patients andproduce recommendations for clinicians on prescribing and monitoring. Work withcare home staff to improve safety of medicines ordering and administration.
Managementof common/minor/self- limiting ailments:
Managing caseload of patients withcommon/minor/self-limiting ailments while working within a scope of practiceand limits of competence.
Signposting to community pharmacy and referring to GPs orother healthcare professionals where appropriate.
Provide patient facing clinics and a telephone helpline forthose with questions, queries and concerns about their medicines in thepractice.
Medicineinformation to practice staff and patients:
Answers relevant medicine-related enquiries from GPs, other practicestaff, other healthcare teams (e.g. community pharmacy) and patients withqueries about medicines.
Suggesting and recommending solutions.
Providing follow up for patients to monitor the effect ofany changes
Telephone medicines support
Provide a telephone help line for patients with questions,queries and concerns about their medicines.
Helpingtackle the issue of unplanned hospital admissions:
Review the use of medicines most commonly associated withunplanned hospital admissions and readmissions through audit and individualpatient reviews.
Put in place changes to reduce the prescribing of thesemedicines to high-risk patient groups.
Managementof medicines at discharge from hospital:
To reconcile medicines following discharge from hospitals, intermediatecare and into care homes, including identifying and rectifying unexplainedchanges and working with patients and community pharmacists to ensure patientsreceive the medicines they need post discharge.
Set up and manage systems to ensure continuity of medicinessupply to high risk groups of patients (e.g. those with medicine complianceaids or those in Care Homes).
Signposting:
Ensure that patients are referred to the appropriatehealthcare professional for the appropriate level of care within an appropriateperiod e.g. pathology results, common/minor ailments, acute conditions, longterm condition reviews etc.
Repeatprescribing:
Produce and implement a repeat prescribing policy within the practice. Manage the repeat prescribing reauthorisation process byreviewing patient requests for repeat prescriptions and reviewing medicinesreaching review dates and flagging up those needing a review.
Ensure patients have appropriate monitoring tests in placewhen required.
Riskstratification:
Identification of cohorts of patients at high risk of harmfrom medicines through pre-prepared practice computer searches. This mightinclude risks that are patient related, medicine related, or both.
Servicedevelopment:
Contribute pharmaceutical advice for the development andimplementation of new services that have medicinal components (e.g. advice ontreatment pathways and patient information leaflets).
Medicinesquality improvement:
Undertake clinical audits of prescribing in areas directedby the GPs, feedback the results and implement changes in conjunction with therelevant practice team.
Implement changes to medicines that result from MHRA alerts,product withdrawal and other local and national guidance.
Auditing practices compliance against NICE technologyassessment guidance.
CareQuality Commission:
Work with the general practice team to ensure the practiceis compliant with CQC standards where medicines are involved.
CollaborativeWorking Relationships:
Participates in the Primary Care Network MultidisciplinaryTeam.
Liaises with ICB colleagues including ICB pharmacists onprescribing related matters to ensure consistency of patient care and benefitin surgeries.
Foster and maintain strong links with all services acrossthe PCN and neighbouring networks.
Explores the potential for collaborative working and takesopportunities to initiate and sustain such relationships.
