We are currently hiring a Clinical Nurse Manager (CNM)-Patient Flow for a leading Nursing Home in County Dublin .

Responsibilities
- Manage patient referrals from acute hospitals and other key referral sources, reviewing referrals and liaising with referral teams and Discharge Coordinators.
- Conduct comprehensive pre-admission assessments to determine patient suitability for Care Centre and the appropriate service.
- Collaborate with the ADON to establish discharge plans and anticipated discharge dates for short-term and rehabilitation admissions.
- Coordinate and facilitate the admission of accepted referrals to Care Centre.
- Act as a case manager for patients as required, in collaboration with the Patient Flow team.
- Ensure compliance with all Healthcare policies, procedures, and standards.
- Maintain accurate, clear, and comprehensive assessment records and referral documentation in line with legislation and organisational requirements.
- Maintain knowledge of and compliance with HIQA standards, including Safer, Better Healthcare.
- Ensure all patient admissions are completed appropriately, with all required documentation accurately prepared and recorded.
- Participate in daily handovers and weekly multidisciplinary team (MDT) meetings.
- Provide nursing unit cover when required to support the effective operation of the Centre.
- Liaise with medical practitioners regarding admissions and prescriptions, ensuring all necessary admission documentation is prepared for the admitting doctor.
- Monitor and follow up on patients' outpatient appointments as required.
- Coordinate and facilitate patient admissions and discharges.
- Refer patients to the Medical Social Worker when appropriate.
- Maintain effective communication and coordination with referral sources, hospitals, discharge agencies, and relevant community services.
- Minimum of 3 years' post-registration experience in a relevant clinical setting, with proven experience managing patient caseloads.
- Hold a recognised Management Qualification at Level 6 or above on the National Framework of Qualifications, or be actively working towards achieving this level.
- Hold current registration with the NMBI and be listed on the live register.
- Demonstrated clinical experience in older people's care or nursing home settings.
- Sound knowledge and practical understanding of HIQA Standards and the Health Act Regulations 2013.
- Excellent command of written and spoken English, with eligibility to work in Ireland.
- Strong communication, interpersonal, and active listening skills.
- Ability to work effectively and collaboratively as part of a multidisciplinary care team.
- Compassionate and caring approach, with a strong commitment to delivering high-quality, person-centred care.
- Basic IT proficiency, including familiarity with Microsoft Excel and other relevant systems.
- Good knowledge and understanding of Home Support applications and the Nursing Home Support Scheme (NHSS/Fair Deal).
- All appointments are subject to satisfactory references, medical assessment, and Garda vetting.
