Job overview The Clinical Coordinator for Children's Palliative Care and Rapid Response provides clinical leadership, operational management and oversight of the service. The post holder ensures safe, responsive and high-quality care for children and young people with life-limiting, life-threatening and complex health needs, supporting care in their preferred setting.

Working with the Clinical Service Lead and Service Manager, the post holder coordinates clinical activity, staffing and resources to meet changing patient needs. The role combines expert clinical practice with leadership, supporting rapid response interventions, symptom management, end of life care and reducing avoidable hospital admissions through proactive assessment and care planning.

Responsibilities include workforce coordination, service performance, staff development, supervision and education. Through partnership working with health, social care and voluntary sector providers, the Clinical Coordinator supports integrated, child-centred care that improves outcomes and patient experience.

The service operates seven days a week from 07:30 to 22:00, requiring flexible working to support urgent and complex care within community settings.

Main duties of the job Provide clinical and professional leadership to the Children's Palliative Care and Rapid Response Team, ensuring safe, effective and high-quality care.

Undertake advanced assessment, care planning and review for children and young people with complex, life limiting and life threatening conditions.

Support symptom management, rapid response interventions and end of life care within community settings.

Lead virtual clinics and proactive care planning to improve outcomes and reduce avoidable hospital admissions.

Work with health, social care, hospice and voluntary sector partners to deliver integrated, child-centred care.

Monitor service activity, quality and performance, identifying risks and leading service improvements.

Support workforce planning, recruitment, rota management and effective resource deployment.

Provide clinical supervision, mentorship, education and training to support staff development.

Working closely with the Clinical Lead and Service Manager, you will provide expert clinical leadership, coordinate service delivery and help shape the future of children's palliative and rapid response care.

About the RoleAs Clinical Co-Ordinator, you will be responsible for the day to day operational coordination of the service, ensuring children and families receive responsive, safe and effective care when they need it most. You will support the allocation of staff and resources, provide expert clinical advice and leadership, and work collaboratively with acute, community and hospice partners to deliver seamless care.

Person specification Qualifications and Training

• RSCN or Registered Nurse Child Branch
• Evidence of recent Post Basic Training
• Community Children's Nursing qualification degree level or willingness to undertake
• ENB 998 (Teaching & Assessing in Clinical Practice) or equivalent
• Independent supplementary Nurse Prescriber or willingness to work towards
• Clinical assessment course/ Advanced Nurse Practitioner.

Experience

• Experience working with children with life limiting/threatening illnesses and End of Life
• Significant experience working as a RSCH/RN Child Branch

Skills and Knowledge

• Ability to manage the team on a day to day basis
• Demonstrate knowledge relating to child protection issues and procedures
• Evidence of assessment skills and use of the nursing process and nurse models
• Ability to demonstrate sound knowledge and application of NMC Code of Conduct and Scope of Professional Care
• Ability to demonstrate knowledge of current legislative changes effecting the NHS and Social Services particularly those affecting Primary Care
• Ability to demonstrate sound knowledge and application of up-to-date clinical trends and practices including evidence based ones particularly concerning Children's Community Nursing
• Ability to teach relatives, patients, clients and staff across all areas of self care and appropriate nursing practice
• Ability to demonstrate a commitment to team working
• Demonstrate an ability to be flexible and able to participate in a changing NHS Environment.
• Ability to learn and apply knowledge appropriately
