Care Coordinator

About the Role

As Care Coordinator, you'll be the vital link between our clients, carers, and healthcare partners. You'll ensure care is delivered smoothly, efficiently, and to the highest standards.

Responsibilities Care Management

• Develop and review individual care plans
• Conduct client assessments
• Match carers to clients based on needs and compatibility
• Monitor care quality and client satisfaction
• Handle complaints and concerns professionally

Team Coordination

• Schedule and roster care staff
• Support carers with complex cases
• Conduct supervision and performance reviews
• Identify training needs

Stakeholder Relations

• Liaise with families, GPs, and healthcare teams
• Build relationships with referral sources
• Represent 360 Living professionally

Requirements

• Understanding of Irish healthcare landscape
• Knowledge of HSE funding processes
• Experience with care management software
• Background in domiciliary care
• Degree in healthcare, social care, or related field
• Minimum 2 years experience in care coordination or management
• Strong organisational and multitasking abilities
• Excellent IT skills including care management systems
• Full driving licence
