Job Description
I am seeking an experienced and compassionate Care Coordinator. This role is responsible for coordinating home support services, ensuring older people receive the right care at the right time, enabling them to remain safely and independently in their own homes.
About the RoleAs Care Coordinator, you will work closely with healthcare professionals, care providers, clients and families to assess, arrange and monitor care supports. You will play a key role in case management, service planning, quality assurance and stakeholder engagement.

- 35 hours per week
- Dun-Laoghaire Based
- Initial 3 month contract with view of extension
- 33.64 per hour
- Coordinate and manage home support packages for older people.
- Liaise with multidisciplinary teams, healthcare professionals and external service providers.
- Monitor service delivery and ensure supports meet assessed needs.
- Manage caseloads, waiting lists and service prioritisation processes.
- Review client and family feedback and support complaint resolution.
- Participate in service improvement, quality assurance and audit activities.
- Prepare reports, maintain accurate records and analyse service data.
- Support collaborative working across community, voluntary and private care providers.
- Deputise for senior management when required.
- Experience working in care coordination, case management, healthcare, social care or a related field.
- Strong stakeholder management skills and experience working across multidisciplinary teams.
- Experience managing competing priorities and resources effectively.
- Knowledge of quality assurance, service improvement and customer service principles.
- Excellent communication, organisational and problem solving skills.
- Strong IT skills, including Microsoft Office applications.
- Access to transport, as travel is required.
- Client focused and passionate about delivering high quality care.
- A strong relationship builder with excellent interpersonal skills.
- Highly organised, proactive and able to work independently.
- Comfortable managing complex situations and making informed decisions.
- Committed to continuous improvement and service excellence.
