Purpose of the role

The role of the Basic Grade Pharmacist will include identification and optimisation of medicine related issues to improve patient quality of care and safety.

Key Duties and Responsibilities
  • Provide clinical pharmacy input into the care of patients in close co operation with medical and nursing staff, and in accordance with departmental resource allocations and prioritisation procedures. This will include chart review and clinical input on all aspects of the medication use process.
  • Work to ensure optimal patient outcomes from drug therapy. This will involve providing help and advice to patients on all aspects of therapy, advising on available schemes to minimise the financial burden on the patient and collaborating with community pharmacists to ensure continuity of supply of medication where necessary.
  • Participate in the dispensary and aseptic unit cover roster as specified in the roster prepared by the Dispensary Manager or Pharmacy Aseptic Unit Manager, in co ordination with the Clinical Pharmacy Services Manager.
  • Complete projects as assigned by managers within the Pharmacy Department.
  • Participate in continuing education and research activities consistent with the post.
  • Participate in the on call and weekend service duty roster.
  • Participate in the pharmacy service as part of a professional, punctual and dedicated team.
  • Create, promote and maintain open communications and healthy working relationships.
  • Maintain the principles of equity, accountability and quality of service in daily work.
  • Ensure the cost effective use of medication and advise on changes in practice likely to affect expenditure or purchasing patterns.
  • Argue for cost effective choices by medical staff and assist in the development of policies, protocols and guidelines designed to deliver optimal care economically.
  • Offer advice and intervene where appropriate to ensure best practice and compliance with guidelines, actively participating in agreed ward rounds.
  • Collect and record daily activity data using the supplied technology to facilitate feedback to management, medical and pharmacy staff.
  • Participate in regular clinical audit to ensure medication use processes are measured, analysed and optimised.
  • Ensure continuity of service by handing over pertinent information to other pharmacists at appropriate times (such as holidays) and keeping the on call pharmacist appraised of any issues deemed appropriate.
  • Ensure all pharmacy staff are kept aware of new developments and practices in their area of responsibility.
  • Perform other duties appropriate to the post as may be assigned by the Head of Pharmacy.
  • Accept cover of other roles which may involve working from an off site location (e.g. Tymon North).
Qualifications & Experience required

Must have

  • Registered as a Pharmacist with the Pharmaceutical Society of Ireland (PSI) or entitled to register.
  • Possess the requisite knowledge and ability (including a high standard of suitability) for the proper discharge of the duties of the office.
Reward & Recognition
  • Remuneration is in accordance with the Department of Health Consolidated Salary Scales, grade code 3247.
  • The appointment is full time, temporary Specified Purpose contract and pensionable.
  • The annual leave entitlement is 27 working days per year. The leave year runs from 1st April to 31st March each year.
  • Normal working hours are 35 worked over 5 days. Contracted hours of work are liable to change between the hours of 8 am to 8 pm over 7 days to meet the requirements for extended day services in accordance with the terms of the Framework Agreement.
