Bank Advanced Clinical Practitioner - Vascular

Advanced Clinical Practitioner to provide clinical leadership within a community-based Multidisciplinary Team (MDT) Vascular clinic across the Oldham area on a 12 months fixed term contract.

The service will involve patients referred by GP for suspected peripheral arterial disease (PAD). The patient journey requires comprehensive vascular assessment, diagnosis (including ABPI) and a management plan, which may include diabetic review and medication changes, lifestyle advice, health promotion and onward referrals where appropriate.

The successful candidate will be an experienced Advanced Clinical Practitioner with expertise in diabetes vascular care. You will have a genuine passion for diabetes management and a commitment to delivering excellent patient centred care within a primary care setting.

Main duties of the job
  • Mobilise the well leg clinic, develop clinical pathways, SOPs and assessment templates.
  • Support training and conduct comprehensive assessments, diagnosis and management of PAD and diabetes.
  • Provide holistic care to patients with diabetes, including monitoring, education and support.
  • Lead, support and train junior staff within MDT clinics and innovative ways to meet individual patient needs.
  • Collaborate with GPs, multidisciplinary teams and local services to optimise patient outcomes.
  • Promote best practice in line with current guidelines.
  • Provide data and analysis on patient outcomes and key performance indicators.
Job responsibilities

Clinical Practice

  • Assess and examine patients with a range of needs through history, physical exam and investigations to confirm diagnosis.
  • Plan, implement and evaluate effective management plans ensuring safe discharge/referral.
  • Provide information and advice on medication regimens, side effects and interactions.
  • Prioritise health problems and intervene appropriately in complex, urgent or emergency situations.
  • Refer and discharge safely with safety netting advice.
  • Triage telephone referrals, calls from clinicians, patients and others into services appropriately.

Communication

  • Utilise sensitive communication styles to ensure patients are fully informed and consent to treatment.
  • Communicate effectively with patients and carers, recognising cultural and communication needs.
  • Anticipate barriers to communication and take action to improve them.
  • Maintain effective communication within the organisation and with external stakeholders.
  • Provide information in an acceptable format recognising difficulties and referring where appropriate.

Non Medical Prescribing

  • Prescribe within scope of practice and comply with NMP governance.
  • Maintain knowledge and skills and prescribe within clinical competencies.
  • Adhere to local and national prescribing guidance.

Person Specification Experience
  • Experience of working as senior clinician.
  • Experience of autonomous practice.
  • Experience of having worked in Primary Care.
  • Ability to lead an MDT to provide high quality patient journey.
  • Experience in leading/managing a team.
  • Experience of working in unscheduled care service.
  • Experience of leading/managing quality improvement.

Qualifications
  • Registered healthcare professional (e.g., first level nurse, paramedic, pharmacist or other AHP) with MSc Advanced Clinical Practice or credentialing qualification.
  • V300 Non Medical Prescriber (gateway dependent) dependent on professional eligibility.
  • One or both of a Diabetic diploma or equivalent experience and a Vascular diploma or equivalent experience.
  • Clinical supervision training and experience.
