Role Responsibilities
  • Mobility Assessments & Care Planning: Conduct thorough mobility assessments, designing tailored exercise programmes for patients based on individual needs.

  • Discharge Planning: Assist in discharge planning to ensure a smooth transition for patients from hospital to home or community care.

  • Supervision & Mentorship: Guide and support junior staff, sharing your knowledge and providing hands-on mentoring to help them develop professionally.

  • Patient Care: Work closely with individuals suffering from conditions such as arthritis, heart disease, diabetes, dementia, musculoskeletal disorders (MSK), and chronic obstructive pulmonary disease (COPD), implementing best practice physiotherapy interventions.

  • Manual Handling & Equipment Provision: Use your expertise in manual handling techniques and provide necessary equipment to enhance patient outcomes.

  • Travel & Equipment Management: As a car driver, ensure the efficient transport of equipment needed for patient care, contributing to positive patient outcomes.

Person Specification

Essential:

  • HCPC registration as a Physiotherapist.

  • Proven experience in hospital ward settings, rehabilitation, and mobility assessments.
