Band 6 Critical Care Follow Up Sister/Charge Nurse

The post holder will work as part of the Critical Care Follow Up Service team, providing specialist follow-up care to patients who have experienced critical illness. Working under the supervision of the Band 7 Service Lead, the post holder will deliver evidence based nursing care to support patients and families through their recovery journey, addressing physical, psychological and social needs following critical care admission.

The post holder will participate in nurse led clinics, conduct telephone consultations, and work collaboratively with multidisciplinary teams across hospital and community settings. The role requires specialist knowledge of post intensive care syndrome and recovery pathways, with a focus on promoting optimal patient outcomes and experience.

The service operates in line with NICE guidance on rehabilitation after critical illness and supports patients from critical care discharge through to community based recovery, typically for patients who have stayed in critical care for 96 hours or more, or those identified as having ongoing needs regardless of length of stay.

Main duties of the job

About the Role:

As part of the nurse led Follow Up Service, you will deliver holistic, evidence based care to patients and families following critical care admission. You will conduct comprehensive physical, psychological and social assessments, support nurse led clinics, provide expert advice to ward teams, and liaise closely with MDT colleagues. You will recognise and manage post intensive care syndrome and ensure timely referrals to psychological and rehabilitation services.

You will work autonomously within defined protocols, contributing to long term ward rounds, care planning and safe patient management. This role requires excellent communication skills, the ability to support patients processing their ICU experience, and confidence in responding to clinical emergencies.

Key Responsibilities:
  • Provide specialist follow up nursing care for patients recovering from critical illness.
  • Conduct comprehensive assessments and deliver evidence based interventions.
  • Support and participate in nurse led clinics and multidisciplinary ward rounds.
  • Offer expert guidance to ward staff on care needs of critical care survivors.
  • Provide psychological support and recognise signs of PTSD, anxiety or depression.
  • Work collaboratively with critical care teams, therapists, psychologists, GPs and community services.
  • Maintain accurate patient documentation and support audit and service evaluation.
  • Contribute to service development, quality improvement and policy implementation.

Person Specification Qualifications
  • Registered Adult Nurse on NMC register
  • Evidence of continuing professional development
  • Critical care qualification (level 6 or above
  • Counselling skills training
Experience
  • Post registration experience working in Critical Care
  • Understanding of multidisciplinary team working
  • Knowledge of rehabilitation principles
  • Experience in patient assessment and care planning
  • In depth knowledge of specialist procedures required for role, with willingness to undertake further training/ education to achieve agreed level of competence.
  • Knowledge of standards and policies associated with role.
  • Experience of working in a multi disciplinary and cross agency work environment. planning processes
  • Demonstrable understanding of clinical governance / patient safety process's - can relate this to the role in relation to their personal/professional accountability
  • Experience in bereavement support
  • Experience in rehab healthcare settings
