Band 6 Complex Discharge Nurse Specialist

This is an opportunity to join the energetic, innovative complex discharge team. It is an exciting time for complex discharge with many new pathways and initiatives being introduced with partnerships with external agencies being forged by the complex team.

You will provide clinical expertise to support safe, effective and timely discharge working directly with internal and external multidisciplinary teams as required.

The Complex Discharge Team is looking for a dynamic, innovative, and enthusiastic experienced nurse to join the team, to ensure the safe, effective, appropriate and timely discharge of patients from hospital. You'll need to be enthusiastic and have a strong sense of purpose and good diplomatic skills in engaging staff. However, you'll also need to be tenacious, confident, calm and persuasive.

As a registered nurse you will have significant experience in an acute hospital care setting, with extensive knowledge of the discharge process through relevant post qualification training and experience.

Main duties of the job

As the Band 6 within the team, you would be supporting the Band 7 and Band 6 Complex Discharge Nurses to ensure the timely completion of complex discharge assessments and service provision whilst having an awareness of complex discharge including but not limited to continuing health care processes including End of life, Mental capacity, DOLS, safeguarding and Discharge to assess.

You will also be involved in the Single Transfer of Care process, undertaking triaging of referrals and decision making regarding the hospital discharge pathways.

You will be a self motivated team player, who will uphold and promote our Trust values. You will need to demonstrate excellent communication skills and be able to confidently liaise with all members of the multi disciplinary teams and external agencies involved in the discharge process and provide support and teaching to the wards, attend MDT and lead and signpost on all complex discharge issues in the trust.

You should be confident in completing holistic, nursing needs based assessments and will also need to be confident liaising with all MDT, patients and their families. This job role includes daily liaison with all clinical ward areas, including emergency department as needed and excellent communication skills are essential for this post.

Person Specification Qualifications
  • Registered Nurse on the NMC register.
  • Maintained relevant continuing professional development in the past 3 years which supports revalidation.
  • Evidence of knowledge of the Hospital Discharge and Community policy.
  • Evidence of knowledge of Discharge to Assess and the hospital discharge pathways.
  • Ability to demonstrate the importance of, and commitment to integrated working.
  • Teaching / assessor qualification.
Experience
  • Ability to act on own initiative with evidence of sound decision making abilities.
  • Ability to demonstrate up to date experience and depth of knowledge and clinical competence.
  • Experience of MDT working.
  • Experience in an acute hospital/community setting.
  • Experience of caseload management.

  • Excellent IT skills and ability to work across multiple systems.
