Job overviewThese new roles support mental health services within Primary Care Networks (PCNs) in Kent, which are groups of GP practices working together locally. Multiple posts are available across the PCNs.
We seek experienced Mental Health Practitioners-nurses, social workers, or occupational therapists-to work within PCNs under the ARRS scheme.
This is an exciting phase as we transform community mental health services, with these roles forming the foundation of new developments. You will work mainly in GP surgeries as part of integrated teams.
Main duties of the jobThe practitioner will be embedded within the PCN Multi-Disciplinary Team, collaborating with secondary mental health services to provide high-quality care and act as a bridge between primary and secondary care.
The role demands a highly competent practitioner capable of autonomous, creative work, with excellent assessment skills and knowledge of local resources, serving as a gatekeeper for referrals to secondary care.

Person specification Experience- Significant experience working with individuals with mental ill health in a community setting
- A minimum of 3 years post-qualification experience; current registration with an appropriate healthcare or social work body
- Post-graduate level knowledge of evidence-based practices in mental health care
- Ability to communicate complex and sensitive information effectively to patients, families, and multidisciplinary teams
