Advanced Practitioner Specialist Palliative Care

Working within the scope of advanced practice the post holder will be an integral part of the Supportive & Palliative Care Team (SPCT) providing expert advice, holistic assessment and management of palliative care needs for adult patients with life limiting conditions. The post holder will lead and deliver a comprehensive education programme to non palliative care specialists to support the provision of safe, high quality care, improve patient outcomes, and reduce length of stay.
Successful applicants without an MSc will be supported to obtain a master's in advanced practice and/or skills and experience in specialist palliative care, if required. The post holder will attain band 8a on completion of MSc in advanced practice and/or after achieving specialist palliative care competencies.
This post is based on the Weston site but will require cross site working at times.
Part time hours would be considered, but a minimum of 30 hours is required.

Main duties of the job- Work as part of the multidisciplinary Supportive and Palliative Care Team (SPCT), practice autonomously, demonstrate high levels of expertise and advanced clinical decision making in the clinical management of patients with palliative care needs.
- Work in conjunction with multiple teams and in partnership with patients and those closest to them to form clinical decisions and complex management plans to improve patient experience and outcomes.
- Utilise expertise within the speciality to develop and lead an education programme to enhance the skills of non palliative care specialists across the Trust, ensuring the provision of high quality care.
- Work across professional, organisational and system boundaries to proactively develop and sustain new partnerships and networks to influence and improve health outcomes and healthcare delivery systems.

Job responsibilitiesAdvanced practice is delivered by experienced, registered health and care practitioners. It is a level of practice characterised by a high degree of autonomy and complex decision making. It is underpinned by a master's level award or equivalent that encompasses the four pillars of clinical practice, leadership and management, education and research, with demonstration of core capabilities and area specific clinical competence. Advanced clinical practice embodies the ability to manage clinical care in partnership with individuals, families and carers, including the analysis and synthesis of complex problems across a range of settings, enabling innovative solutions to enhance people's experience and improve outcomes.
Person Specification Qualifications- Registered healthcare professional
- Evidence of formal ongoing professional development at master's level
- Teaching qualification or proven equivalent experience
- Completed Advanced Practice Masters and equivalent mapped to apprenticeship standard
- NMP

- Significant experience at band 7 relevant to post
- Proven leadership skills
- Proven evidence of educational support to others
- Experience of change management as part of leadership role
- Ability to organise and prioritise own workload and that of junior colleagues

- Familiar with current local and national issues related to specialty
