Advanced Practitioner - Lipids Service

Clinical Responsibilities
- Independently assess, diagnose, and manage patients needing lipid optimisation.
- Identify and review those with hyperlipidaemia, familial hypercholesterolaemia (FH), established cardiovascular disease (CVD), or elevated risk.
- Support the full lipid management pathway: patient identification, clinical assessment, risk stratification, medication initiation and optimisation, and ongoing monitoring and review.
- Prescribe lipid lowering therapies (oral and injectable) in line with competence, NICE guidance, and local pathways.
- Monitor treatment response, safety, adherence, and outcomes; interpret investigations to inform decisions; and refer complex cases when required.
- Maintain accurate clinical records.
- Provide expert advice on cholesterol management, CVD prevention, lifestyle modification, diet, exercise, smoking cessation, and medication adherence.
- Promote shared decision making and personalised care, improving patient understanding of risk and treatment benefits.
Service Delivery & Leadership
- Support delivery of the Lipids programme across practices and neighbourhood teams.
- Contribute to pathway development, governance, and service improvement.
- Participate in MDT working across primary and secondary care, support audit and evaluation, and act as a clinical lipid management resource within the multidisciplinary team.

Job responsibilitiesJob Purpose
The Advanced Practitioner (AP) will lead and support the end-to-end delivery of the Lipids Programme, focusing on the identification, assessment, treatment, prescribing, and ongoing management of patients requiring lipid optimisation.
The postholder will independently manage patients within agreed clinical pathways and will support the initiation, prescribing, administration, monitoring, and ongoing management of lipid lowering therapies in line with NICE guidance, local pathways, and national cardiovascular prevention priorities.
The role will work collaboratively across primary care, neighbourhood teams, and wider healthcare partners to improve cardiovascular prevention outcomes and support proactive population health management.
Key Responsibilities
Clinical Responsibilities
Independently assess, diagnose, and manage patients requiring lipid optimisation interventions.
Identify and review patients with:
- Familial Hypercholesterolaemia (FH)
- Elevated cardiovascular risk
Assist in the end to end lipid management pathway, including:
- Clinical assessment
- Medication initiation and optimisation
- Monitoring and follow up
Independently prescribe lipid lowering therapies in line with clinical competence and prescribing regulations.
Initiate, prescribe, administer, and manage oral and injectable therapies in accordance with NICE guidance and locally agreed clinical pathways.
Monitor treatment response, safety, adherence, and clinical outcomes.
Interpret pathology results and other relevant investigations to support clinical decision making.
Escalate or refer complex patients appropriately where specialist input is required.
Maintain accurate and contemporaneous clinical records.
- Provide advanced clinical advice and education relating to:
- Cholesterol management
- Cardiovascular disease prevention
- Diet, exercise, and smoking cessation
- Medication adherence and treatment options
- Promote shared decision making and personalised care planning.
- Support patients to improve understanding of cardiovascular risk and treatment benefits.
Service Delivery & Leadership
- Support the development and implementation of the Lipids programme across participating practices and neighbourhood teams.
- Contribute to pathway development, clinical governance, and service improvement initiatives.
- Participate in MDT meetings and collaborative working across primary and secondary care.
- Support audit, data collection, and pilot evaluation activities.
- Act as a clinical resource for lipid management within the wider multidisciplinary team.
- Registered healthcare professional with current professional registration.
- Independent & Supplementary Prescriber qualification.
- Working to MSc level or above with advanced practice modules.
- Demonstrable experience in primary care, cardiovascular disease prevention, and long term condition management.
- Experience managing lipid lowering therapies and cardiovascular risk.
- Knowledge of NICE guidance relating to lipid management.
- Excellent clinical assessment and decision making skills.
- Strong communication and patient engagement skills.
- Ability to work autonomously and within multidisciplinary teams.
- Experience leading or supporting programmes or service transformation projects.
- Experience in injectable therapy administration.
- Knowledge of population health management approaches.
- Experience undertaking clinical audit and quality improvement work.
- Advanced clinical assessment skills.
