Weare seeking an experienced ANP/ACP to join our team, which operates across 2 sites and provide primary care services to approximately 11,200patients.
Pay and hours are negotiable (looking for between 24 to 37.5 hrs per week)
Main duties of the jobWe are seeking an experienced Advanced Clinical Practitioner (ACP) / Advanced NursePractitioner (ANP) who provideshigh-quality, patient-centred care within the general practice setting. Workingautonomously within your professional boundaries, you will be responsible forthe assessment, diagnosis, treatment, and evaluation of patients presentingwith undifferentiated and undiagnosed conditions, as well as those with chronicillnesses.
You will undertake comprehensive clinicalassessments, take patient histories, and make safe, evidence-based decisionsregarding treatment and care plans. You will exercise advanced clinicalreasoning and professional judgement to deliver effective care, ensuring a highstandard of service that supports both the immediate and long-term needs of thepractice population.
This role requires working both independently andcollaboratively as part of the multidisciplinary team to ensure continuity,safety, and excellence in patient care within the primary care environment.

We are a friendly, supportive and innovative team led by 2 forward thinking GP Partners, and includes GPs, ACPs, ANP's, clinical pharmacists, FCPs, PAs, social prescribers, and a highly experienced nursing team.

Job responsibilitiesWorking across the 2 sites, you will;
- Undertake consultationswith patients taking a comprehensive patient history, carry out physicalexamination and use their expert knowledge and clinical judgement to identifythe potential diagnosis.
- Exercise a higher degree ofpersonal and professional autonomy using advanced clinical practice skills toinclude clinical interventions.
- Refer patients forinvestigations when necessary/appropriate.
- Make a final diagnosis,decide on and carry out treatment, including the prescribing of medicine orreferring to primary or secondary care specialist services within local agreedpathways and protocols as appropriate.
- Ensure that all treatmentsand care provided are based on best practice/evidence based care.
- Plan and provide skilledcompetent care to meet the patients health and social care needs, involvingother members of the healthcare team as appropriate.
- Work withmultidisciplinary and multiagency teams in order to ensure patient needs aremet
- Asses and evaluate, withthe patient, the effectiveness of the treatment and care provided and makechanges as needed
- Use independent/supplementaryprescribing skills for benefits of patient care
- Work independently and aspart of a team
- Take responsibility andbe accountable for managing own workload, risk assessment and management,acting within professional codes and appropriately
- Liaise with the clinical team leaderfor advice and support.
- Undertake other dutieswhen required, such as audits or administrative tasks.
- Maintain contemporaneousrecords of the consultation and action using the computer software and wherenecessary written records, in line with NMC guidance and local policies,guidelines and procedures.

Person Specification Experience- Working within primary care
- Registration as a healthcare professional (e.g. nurse, paramedic, or allied health professional)
- Masters degree in Advanced Clinical Practice
- Digital Badge
