Accountable to PCN Clinical Director

We are lookingfor an Advanced Nurse Practitioner or Advanced Clinical Practitioner who ispassionate about providing excellent care.Is compassionate and a real team player.You will be working alongside an approachable and supportive GP.

Main duties of the job To work as an autonomous practitioner, workingas part of the practice multidisciplinary team, delivering care within theirscope of practice to the patient population. Manage patients presenting with a range of acute andchronic medical conditions, providing subject matter expert advice to patients

The Advanced Nurse/Clinical Practitioner will be responsible forseveral clinical areas such as health promotion, minor illness, as well assupporting the management team in the reviewing of clinical policies andprocedures.

The post holderworks at the level of Advanced Nurse/Clinical Practice and has developed andcan evidence the underpinning competencies applicable to the specialty orsubject area, i.e., the knowledge, skills and behaviours appropriate to the healthcareprofessionals (HCP) setting and job role.

The roleprovidesan opportunity for patients to avoid the need to be referred to a GP, therebyenabling them to receive timely care and negate unnecessary delay in receivingtreatment.

An ANP/ACPin primary care can develop close, long-term relationships with their patientsand work in partnership with them to achieve optimum health. They are autonomous in making decisions basedon assessment, diagnosis and interpretation of test results. They can independently prescribe appropriatemedication, evaluate or refer to other specialists if necessary.

About us The TardisSurgery is one of 7 practices within the Moorlands Rural PCN.

We currentlyhave 5,900 patients on the practice list; the surgery is set in a semi-ruraltown,

We provideservices to two nursing homes and the Moorlands Neurological centre providingweekly visits to the homes and monthly MDTs. We are also a dispensing practiceserving our patients who live more than a mile away from a community Pharmacy.

We are a veryfriendly team and try to support our staff to continue to develop.

Our staffingconsists of:

2 GP Partners,ANP, Practice Nurse, HCSW, Dispensers, Clinical coder, Clinical Administrator,Reception/Admin team and a management team.

We also havesupport from our Moorland Rural PCN ARRS team which consists of:

FCP, MHP, STRworker, Pharmacy team, Care coordinator, HWBC, counsellor, and socialprescribers.

We engageclosely with our local community getting involved with various activities e.g.supporting the local High school with careers days, primary school wellbeingmorning etc.

We have a very committedPPG who work alongside us to ensure we are delivering high quality personcentred care.

Job responsibilities TheAdvanced Nurse/Clinical Practitioner(ANP/ACP) has been assessed ascompetent in practice using their expert clinical knowledge and skills. Theyhave the freedom and authority to act and make autonomous decisions in theassessment, diagnosis and treatment of patients.

The postholder works at the level of advanced clinical practice and has developed andcan evidence the underpinning competencies applicable to the specialty orsubject area, i.e., the knowledge, skills and behaviours appropriate to the healthcareprofessionals (HCP) setting and job role.

An ACP in primary care can develop close, long termrelationships with their patients and work in partnership with them to achieveoptimum health. They are autonomous inmaking decisions based on assessment, diagnosis and interpretation of testresults. They can independentlyprescribe appropriate medication, evaluate or refer to other specialists ifnecessary.

Primary responsabilities

Thefollowing are the core responsibilities of the Advanced Nurse/Clinical Practitioner.

There may be, on occasion, a requirement tocarry out other tasks; this will be dependent upon factors such as workload andstaffing levels:
  • Manage patients presenting with a range of acute andchronic medical conditions.
  • Be an extended and supplementary prescriber, adhering toguidance.
  • Support patients in the use of their prescribed medicinesor over the counter medicines (within own scope of practice), reviewingannually as required.
  • Contribute to practice/PCN targets (QOF, etc.) complyingwith local and regional guidance.
  • Identify manage and support patients at risk ofdeveloping long term conditions, preventing adverse effects to the patientshealth.
  • Request pathology services as necessary.
  • Process and interpret pathology and other test results asrequired.
  • Maintain accurate clinical records in conjunction withlegislation.
  • Support the team in dealing with clinical emergencies.
  • Support the clinical team with all safeguarding mattersin accordance with local and national policies.
  • Practise in compliance with their respective codeof professional conduct and within their scope of practice, being responsibleand accountable for their decisions, actions, and omissions at this level of practice.
  • Demonstratea critical understanding of their broadenedlevel of responsibility and autonomy and the limits of own competence andprofessional scope of practice, including when working with complexity, risk, uncertainty,and incomplete information.
  • Act on professional judgement about when to seek help,demonstrating critical reflection on own practice, self-awareness, emotional intelligence, andopenness to change.
  • Work in partnership with individuals, families, and carers, using arange of assessment methods as appropriate.
  • Demonstrate effective communication skills, supportingpeople in making decisions, planning care, or seeking to make a positive change.
  • Use expertise and decision-making skills to informclinical reasoning.
  • Exercise professional judgement to manage risk.
  • Be a professional role model for the organisation.
  • Actively engage in peer review to inform own and otherspractice, formulating and implementing strategies to act on learning and make improvements.
  • Actively seek feedback and involvement from individuals,families, carers, communities, and colleagues in the co-production of serviceimprovements.
  • Critically assess and address own learning needs,negotiating a personal development plan that reflects the breadth of ongoingprofessional development across the four pillars of advanced clinical practice(clinical practice, leadership and management, education, and research).
  • Chaperonepatients where necessary.
  • Deliveropportunistic health promotion where appropriate.
Secondary resposabilities

Inaddition to the primary responsibilities, the Advanced Nurse/ClinicalPractitioner may be requested to:
  • Participatein local initiatives to enhance service delivery and patient care.
  • Supportand participate in shared learning within the organisation.
  • Developan area of specialist interest, taking the lead within the organisation.
  • Continuallyreview clinical practices, responding to national policies and initiativeswhere appropriate.
  • Participatein the review of significant and near-miss events applying a structuredapproach i.e. root cause analysis (RCA).
  • Commensuratewith their role, support the development of clinical services within theorganisation, liaising with external agencies and professional organisations asrequired.
  • Develop practice administrative and clinical protocolsin line with the needs of the patient and current legislation.
