Anopportunity has arisen to join Padiham Group Practice. The practice has anexcellent reputation as a friendly, hardworking and high achieving accreditedtraining practice. The practice have been rated Good by CQC, with consistent,positive GP experience on the patient survey results which demonstrate thestrong patient centred customer focus and efficiency of the teams. There are two positions of up to 30 hours per week. Salary is negotiable depending on experience.
Main duties of the jobWe arecurrently looking for an Advanced Nurse Practitioner or Advanced ClinicalPractitioner to join our clinical team.
We arelooking to employ and ANP or ACP for four days with the following:
Mastersdegree level in the relevant area of expertise;
Workingat a masters level aware or equivalent that encompasses the four pillars ofclinical practice, leadership and management, education and research, withdemonstration of core capabilities and area specific clinical competencies
ForAdvanced Nurse Practitioner have either graduated from a Centre for AdvancingPractice accredited MSc advanced practice programme or completed the CentresePortfolio (supported) Route; both pathways enable eligibility for anAdvanced digital badge issued by the Centre, demonstrating recognisededucational and experiential preparation in advanced practice
Assess and triagepatients, including same day triage, and as appropriate provide definitivetreatment
You will beresponsible for the clinical assessment, treatment and or referral or givingadvice as appropriate to both adults and children presenting with a widespectrum of clinical conditions
Manageundifferentiated undiagnosed condition and identify red flags and underlyingserious pathology and take appropriate action
Usecomplex decision making to inform the diagnosis, investigation, completemanagement of episodes of care within a broad scope of practice

Job responsibilitiesThefollowing are the core responsibilities of the Advanced Nurse Practitioner.

Practise in compliance with their respectivecode of professional conduct and within their scope of practice, beingresponsible and accountable for their decisions, actions and omissions at thislevel of practice
Demonstrate a critical understanding of theirbroadened level of responsibility and autonomy and the limits of own competenceand professional scope of practice, including when working with complexity,risk, uncertainty and incomplete information
Act on professional judgement about when toseek help, demonstrating critical reflection on own practice,self-awareness, emotional intelligence, and openness to change
Work in partnership withindividuals, families and carers, using a range of assessment methods asappropriate
Demonstrate effective communication skills,supporting people in making decisions, planning care or seeking to make apositive change
Use expertise and decision-making skills toinform clinical reasoning
Initiate, evaluate and modify a range ofinterventions which may include prescribing medicines, therapies, lifestyleadvice and care
Exercise professional judgement to managerisk
Managepatients presenting with a range of acute and chronic medical conditions,providing subject matter expert advice
Implementand evaluate individual specialised treatment plans for chronic diseasepatients
Identify,manage and support patients at risk of developing long-term conditions,preventing adverse effects to the patients health
Be a professional role model for theorganisation
Actively engage in peer review to inform ownand others practice, formulating and implementing strategies to act onlearning and make improvements
Actively seek feedback and involvement fromindividuals, families, carers, communities and colleagues in the co-productionof service improvements
Critically assess and address own learningneeds, negotiating a personal development plan that reflects the breadth ofongoing professional development across the four pillars of advanced clinicalpractice clinical practice, leadership and management, education andresearch.
Maintainaccurate clinical records in conjunction with extant legislation and to ensureSNOMED CT codes are used effectively
Engagein an effective call/recall system
Critically appraise and synthesise theoutcome of relevant research, evaluation and audit, using the results tounderpin own practice and to inform that of others
Engage in robust governance systems and contribute to practice targets QOF,etc., complying with local and regional guidance
Chaperonepatients where necessary
Supportthe team in dealing with clinical emergencies
Supportthe team with all safeguarding matters in accordance with local and nationalpolicies
Understandpractice and local policies for substance abuse and addictive behaviour,referring patients appropriately
Deliveropportunistic health promotion where appropriate
Person Specification Qualifications- Masters degree level in the relevant area of expertise
