Job Description
We have recently recruited an ANP and now we are looking for another dedicated and experienced Advanced Nurse Practitioner (ANP) to join them in our friendly and forward-thinking GP practice.
Monday 8.30-1pm or longer
Friday 8.30-1pm or longer

This is a key clinical role, supporting same-day patient access and delivering high-quality care through advanced assessment, diagnosis, and treatment. You'll work both independently and collaboratively within a supportive multi-disciplinary team, helping to shape and improve primary care services for our local community.
Main duties of the job As an Advanced Clinical Practitioner (ACP), you will provide high-quality, patient-centred care within a busy GP practice setting and home visits. Acting within your professional boundaries, you will manage patients presenting with a range of acute and chronic conditions from initial consultation through diagnosis, treatment, and follow-up.
You'll work closely with the duty GP to deliver timely same-day access, including clinical assessment and triage of patients with urgent needs. The role combines autonomous clinical practice with collaborative team working to ensure effective decision-making and continuity of care. If you have a special interest in care of the elderly / care home patients this can also be accommodated.
You'll be responsible for prescribing and managing medications, supporting health promotion, and contributing to the development of practice protocols and patient pathways. You'll also play a key role in mentoring junior staff and supporting ongoing clinical and quality improvement initiatives.
This is a varied and rewarding role for a motivated and experienced nurse or paramedic with advanced clinical skills, a commitment to holistic patient care, and a passion for service development in primary care.

Job responsibilities Job Summary
The post holder is an experienced ANP who, acting within their professional boundaries, will providecare for the presenting patient from initial history taking, clinicalassessment, diagnosis, treatment and evaluation of care. They will demonstratesafe, clinical decision-making and expert care, including assessment anddiagnostic skills, for patients within the general practice. The post holderwill demonstrate critical thinking in the clinical decision-making process.They will work collaboratively with the general practice team to meet the needsof patients, supporting the delivery of policy and procedures, and providingnurse leadership as required. To work at this level, NMC requirements foradvanced practice must be met.
The post holder will work closely with theduty/on-call GP daily to support access to same-day care for patientspresenting with acute needs, ensuring timely and collaborative clinicaldecision-making.

Key Responsibilities
Clinical Practice
Assess,diagnose, plan, implement and evaluate treatment/interventions and carefor patients presenting with an undifferentiated diagnosis
Clinicallyexamine and assess patient needs from a physiological and psychologicalperspective, and plan clinical care accordingly
Assess,diagnose, plan, implement and evaluate interventions/treatments forpatients with complex needs. Proactively identify, diagnose and managetreatment plans for patients at risk of developing a long-term condition(as appropriate)
Diagnose andmanage both acute and chronic conditions, integrating both drug- andnon-drug-based treatment methods into a management plan
Prescribeand review medication for therapeutic effectiveness, appropriate topatient needs and in accordance with evidence-based practice and nationaland practice protocols, and within scope of practice
Work withpatients to support compliance with and adherence to prescribedtreatments. Provide information and advice on prescribed orover-the-counter medication, side-effects and interactions
Prioritisehealth problems and intervene appropriately in complex, urgent oremergency situations, including initiation of effective emergency care
Supportpatients to adopt health promotion strategies that promote healthylifestyles and apply principles of self-car
Collaboratein real-time with the duty/on-call GP to triage, assess, and managepatients requiring same-day access, ensuring continuity andappropriateness of care
Provideclinical leadership and input in supporting the access/navigation teamwith prioritisation and triage of acute, same-day patient presentations
Communication
Utilise and demonstratesensitive communication styles to ensure patients are fully informed andconsent to treatment
Communicatewith and support patients who are receiving bad news
Communicateeffectively with patients and carers, recognising the need for alternativemethods of communication to overcome different levels of understanding,cultural background and preferred ways of communicating
Anticipatebarriers to communication and take action to improve communication
Maintaineffective communication within the practice environment and with externalstakeholders
Act as anadvocate for patients and colleagues
Ensureawareness of sources of support and guidance (e.g. PALS) and provideinformation in an acceptable format to all patients, recognising anydifficulties and referring where appropriate
Support andhelp develop the patient navigation team
Recognise and work within own competence and professional code of conductas regulated by the NMC.Produce accurate, contemporaneous and complete records of patientconsultation, consistent with legislation, policiesand procedures
Priorities, organise and manage own workload in a mannerthat maintains and promotes quality
Deliver care according to NICE guidelines and evidence-based care
Assess effectiveness of care delivery through self and peer review, bench marking and formal evaluation
Initiate and participate in the maintenance of quality governance systemsand processes across theorganisation and its activities.
Utilise the audit cycle as a means of evaluating the quality of the work of self and the team, implementing improvements where required.
In partnership with other clinical teams, collaborate on improving the quality of health care byresponding to localand national policies and initiatives as appropriate.

Support and participate in shared learning across the practice and wider organisation.
Use a structured framework (eg root-cause analysis) to manage, review and identifylearning from patientcomplaints, clinical incidents and near-miss events
Assess the impact of policy implementation on care delivery.
Understand and apply legal issues that support the identification of vulnerable and abused childrenand adults and be aware of statutory child/vulnerable patients health procedures and local guidance.
