Main duties of the jobYou willbe an experienced registered Advanced Nurse Practitioner who, acting withintheir professional boundaries, will provide care for patients registered at thepractice from initial history taking, clinical assessment, diagnosis,treatment, evaluation of care, prescribing of medication and development ofmanagement plan. The role will involvecare planning and working in the community with the practices patients who maybe housebound, vulnerable adults or residents in care/residential homes.
Youwill demonstrate safe, clinical decision-making and expert care, includingassessment and diagnostic skills, for patients within the general practice. Thepost holder will demonstrate critical thinking in the clinical decision-makingprocess You will work collaboratively with the general practice team to meet theneeds of patients, support the delivery of policy, procedures, future practicestrategy and provide clinical leadership as required.

Our clinical team is diverse including GPs, a Practice Paramedic, Pharmacists, Advanced Clinical Practitioners, Advanced Nurse Practitioners, Practice Nurses, Nurse Associates, a Physician Associate and highly skilled Health Care Support Workers, working closely with a range of additional roles including an Acute Visiting Service, Mental Health Practitioners and First Contact Physiotherapists
To allow more time to care, our clinical team is underpinned by a very experienced team of receptionists and administrators, as well as a range of inhouse corporate functions covering themes such as human resources, information technology, finance, administration and management support and communications
Job responsibilitiesAssess and treat patientspresenting with acute illness or for review of chronic disease conditions.
Undertake homevisits for acute illness or chronic disease management. Reviewing care plans and liaising withpatients, carers, relatives and other healthcare professionals to support aholistic package of care.
Diagnose, plan,implement and evaluate treatment/interventions and care for patients presentingwith an undifferentiated diagnosis
Clinicallyexamine and assess patient needs from a physiological and psychologicalperspective, and plan clinical care accordingly
Assess, diagnosis,plan, implement and evaluate interventions/treatments for patients with complexneeds
Proactivelyidentify, diagnose and manage treatment plans for patients at risk ofdeveloping a long-term condition (as appropriate)
Diagnose andmanage both acute and chronic conditions, integrating both drug- andnon-drug-based treatment methods into a management plan
Prescribe andreview medication for therapeutic effectiveness, appropriate to patient needsand in accordance with evidence-based practice and national, local and practiceprotocols and formularies
Work withpatients and their carers in order to support compliance with and adherence toprescribed treatments.
Provide a telephone triage service,assessing patients health needs and management plans. Implement and evaluate clinical interventionsto meet those needs. Record summary ofdiscussion, intervention and actions on to the clinical system.
Provide information and advice onprescribed or over-the-counter medication on medication regimens, side-effectsand interactions
Prioritise health problems andintervene appropriately to assist the patient in complex, urgent or emergencysituations, including initiation of effective emergency care
Support patients to adopt healthpromotion strategies that promote healthy lifestyles, and apply principles ofself-care
Support and manage health needs ofwomen presenting for family planning or sexual health consultation
Assess, identify and refer patientspresenting with mental health needs in accordance with the NSF for MentalHealth
Implement and participate invaccination and immunisation programmes for both adults and children
Person Specification Experience- Minimum 5 years post registration experience
- At least 3 years recent primary care / community nursing experience
- Nurse-led management of minor illness, minor ailments and injuries
- Nurse-led triage including telephone triage
- Home visit assessment of patients
- Compiling protocols and clinical guidelines
- Leadership in quality initiatives such as clinical benchmarking
- Audit
- Independent Nurse Prescribing or willingness to undertake and complete nurse prescribing training
- Project Management
- Working with community development initiatives
- Health needs assessment
- Sexual Health including fitting / removal of IUCD and LAC
- Minor Surgery
- Extended & Supplementary Prescriber
- Advanced Clinical Practice Skills ANP Qualification or equivalent evidence to demonstrate skills
- In-depth knowledge and management of patients with long term conditions/chronic diseases
- Management of patients with complex needs
- Accountability of own role and other roles in a nurse-led service
- Local and national policy / Wider health economy
- EMIS clinical system
- Clinical governance issues in primary care
