When you join the multidisciplinary team in ourOut Of Hours (OOH) and ClinicalAssessmentservice (CAS)inDevonyou can.
In Devon OOH we cover areas across the county; Exeter, Tiverton, Honiton, Okehampton, Newton Abbot, Totness, Barnstaple and Plymouth.
We are looking to add some permanent members of staff to cover the Saturdays and Sunday shifts 08.00 - 14.00 and 14.00 - 20.00 in Exeter.
We can also offer additional hours in our OOH service, from18.30-08.00 Monday to Friday and 24 hours over the weekend.We can offer a range of sessions to allow you to flexibly choose the right shift pattern for you.
This is an ideal opportunity for an ambitious and committedAdvanced Clinical / Nurse Practitioner (ACP) / (ANP)who can independently prescribe. It's a chance to simplify and speed up the way patients receive care - by assessing them, planning their care, administering treatment and discharging or referring them to other healthcare professionalsvia primary care centres and home visits. You will also be using ourelectronic prescribing system.
This is a permanent part time opportunity. Shift pattern can be discussed at interview but will be weekend based while working from site in Stratus House, Exeter.
What you'll be doing
- Deliver comprehensive clinical assessment, diagnosis, and treatment for patients presenting with urgent healthcare needs.
- Conduct consultations from the clinical base, providing face-to-face care in a fully equipped environment.
- Undertake home visits to patients requiring assessment or treatment in their own residence, offering a personalized and compassionate approach.
- Manage telephone triage, providing expert clinical advice, risk assessment, and directing patients to appropriate care pathways.
- Work autonomously while collaborating closely with a multidisciplinary team to ensure seamless, patient-centred care.
- Maintain accurate clinical records and adhere to Practice Plus Group policies and NHS guidelines to uphold high standards of care.
- Collaborating with GPs, paramedics, mental health professionals, and other clinicians to ensure integrated care across the region.
What we'll look for in you
- Registered Nurse / Paramedic with valid NMC / HCPC registration and MSc in Advanced Clinical Practice or equivalent.
- Experience within Out of Hours, Primary Care GP Surgery, or Urgent Treatment Centre.
- Holder of V300 / Non-Medical Prescribing qualification and minimum 6 months experience post qualification.
- Over 3 years working autonomously.
- Ability to see and treat patients of all ages, including 0 to 2 years old.
- Confident clinical decision-maker with strong assessment skills.
- Excellent communicator with a patient-centred approach.
- Flexible, self-motivated, and committed to ongoing professional development.
