About this jobMersey Care is recruiting an ACP for the bank. This is an exciting opportunity to practice at an advanced level in a fast paced environment on bank shifts. You will work autonomously, be highly motivated and deliver an advanced level of clinical practice within the recognised remit, working within local and nationally agreed frameworks for Advanced Clinical Practice (ACP). The post requires excellent communication and interpersonal skills, the ability to manage challenging situations, and a commitment to quality, mentorship and clinical supervision.
The Advanced Clinical Practitioner on duty will advise and support clinical decision making, coordinate escalation, and oversee patient flow to ensure a smooth experience. The role includes supervising junior team members, developing competencies, and leading clinical audits.
Mersey Care provides a wide range of specialist services across the North West, North Wales and the Midlands, including mental health, learning disability, addiction and brain injury care. Responsibilities- Be professionally and legally accountable for all work undertaken to practice at an advanced level of professional autonomy and accountability that is within Trust policies and the Nursing and Midwifery Code of Conduct (2008) or the Health & Care Professions Council (HCPC).
- Deliver and participate in the clinical care of patients accessing the walk in centre/Urgent Treatment Centres.
- Prioritise workload and clinical interventions as appropriate.
- Provide advanced and comprehensive health assessment based on specialist body of knowledge.
- Appropriately assess, examine, investigate, diagnose and treat patients, ensuring safe management and appropriate referral or discharge.
- Ensure patients are informed and consent is gained prior to treatment, investigation, and management.
- Take responsibility for the supervision and development of junior members of the team on shift.
- Identify the need for appropriate diagnostic tests, interpret results and act on any findings requiring clinical intervention or management.
- Identify and intervene proactively when circumstances contribute to an unsafe environment for patients, staff or relatives, escalating these to the health and safety team/safeguarding team.
- Act as an expert resource to others across primary and secondary care, sharing knowledge and expertise.
- Contribute to the development and improvement of systems and processes that facilitate patient flow, facilitating timely discharge and preventing hospital admission.
- Improve the quality of the patient experience by identifying and meeting individual clinical needs, including issues related to child protection and vulnerable adults.
- Adhere to CPD requirements of the Trust to maintain evidence of ongoing competence.
- Maintain up to date patient records in accordance with Trust policy and professional bodies' standards.
- Adapt specialist clinical knowledge and skills to different clinical settings, influencing service delivery and patient care.
- Promote and adhere to the principles and practice of the Trust's clinical governance framework.
- Contribute to the review and development of clinical patient pathways and the overall service, ensuring safety.
- Adhere to Trust infection and prevention control policies and procedures.
- Maintain up to date qualifications in relation to Intermediate Life Support (PILS) and lead on the review and implementation of appropriate NICE guidance.
- Support junior staff with clinical/therapy competencies.
- Act as a key link for other teams within the service line and for educating staff around a specific area of expertise.
- Lead on clinical audits.
- Act as a link for secondary care to discuss potential patient admissions through collaborative working.
- Completed the Independent Non Medical Prescribing course and registration with the NMC or HCPC.
- Demonstrated ability to undertake non medical prescribing in accordance with the Trust non medical prescribing policy.
- Strong communication and interpersonal skills, with the ability to manage challenging and difficult situations.
- Excellent history taking, extended assessment, clinical examination and clinical diagnosis skills.
- Ability to identify and interpret appropriate diagnostic tests, and act on results requiring intervention.
- Knowledge of and adherence to local and national frameworks for Advanced Clinical Practice.
- Commitment to continuous professional development (CPD).
