Job Description

£10k per annum

Are you an experienced Advanced Clinical Practitioner looking to work in an outstanding CQC-rated Trust?

You will be an established, autonomous, and confident advanced practitioner with respect to complex clinical reasoning. Your role will involve providing expert clinical advice, leadership, and support, ensuring the needs of the service are met by exercising high levels of clinical judgment, critical analysis, and advanced decision-making skills. You will have a keen interest in Older Persons inpatient medical and rehab and End of Life care.

Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check, which the Trust will cover. Applicants who subscribe to the DBS update service can present a valid DBS certificate instead of requiring a new check.

The main duties include planning and managing complete episodes of care, undertaking independent comprehensive assessments, formulating diagnoses and management plans, and making complex clinical decisions regarding service user management and outcomes.

Undertake comprehensive holistic assessments of patients and carers with highly complex needs, planning, implementing, and evaluating care according to changing healthcare needs and risks.

Advise and communicate with hospitals, primary, social, and community care teams to ensure seamless transfer and continuity of care.

Conduct audits and service evaluations to inform improvements, support colleagues in research and clinical audits, and participate in surveys as required.
