Job Description
Overview

Job Purpose The Advanced Clinical Practitioner (ACP) will practise at an advanced level within Heaton Moor Medical Group, delivering high-quality, patient-centred care to adults and children across urgent same-day presentations and long-term condition management.

The role is underpinned by the Four Pillars of Advanced Clinical Practice, requiring a high level of autonomy, advanced clinical expertise, leadership, educational contribution, and engagement in research and service improvement.

Responsibilities
  • Pillar 1: Clinical Practice The ANP will provide expert clinical care to adults and children, managing acute, and chronic conditions within an agreed scope of practice.
  • Independently assess, diagnose, and manage adults and children presenting with urgent and on-the-day clinical problems
  • Manage long-term conditions in adults and children, including reviews, medication optimisation, and monitoring
  • Undertake comprehensive history taking, physical examination, and advanced clinical assessment.
  • Formulate differential diagnoses and evidence-based management plans
  • Prescribe independently within scope of practice and competence, including paediatric prescribing where appropriate
  • Request, interpret, and act upon investigations where appropriate
  • Recognise and manage clinical risk, deterioration, and safeguarding concerns
  • Provide safety-netting, follow-up, and appropriate escalation of care
  • Deliver face-to-face, telephone and home visit consultations as required
  • Maintain accurate clinical documentation
  • Pillar 2: Leadership & Management Key Responsibilities Act as a senior clinical decision-maker for both adult and paediatric presentations within primary care
  • Provide clinical leadership within the multidisciplinary team (MDT)
  • Support service delivery for urgent, same-day, and long-term condition
  • Participate in clinical governance activities, including audit, significant event analysis, and safeguarding reviews
  • Support development and implementation of clinical pathways and protocols
  • Pillar 3: Education Key Responsibilities Act as a role model for advanced clinical practice across adult and paediatric care
  • Support and mentor clinicians, trainees, and students
  • Contribute to teaching related to minor illness, paediatric assessment, and long-term condition management
  • Maintain own competence through CPD, reflective practice, and clinical supervision
  • Contribute to appraisal and peer learning activities
  • Pillar 4: Research & Quality Improvement Key Responsibilities Use research evidence and national guidance to inform adult and paediatric clinical practice
  • Participate in audit and quality improvement initiatives
  • Support evaluation of outcomes related to urgent care and long-term conditions
  • Promote evidence-based innovation and service improvement
  • Contribute to the development and review of local clinical guidelines
  • Professional Responsibilities Maintain professional registration and revalidation
  • Practise within professional codes of conduct and agreed scope of practice
  • Maintain appropriate professional indemnity
  • Always adhere to safeguarding children and adults policies
  • Comply with infection control, information governance, and organisational policies
  • Engage in regular appraisal, supervision, and continuing professional development
