Advanced Clinical Practitioner

Location: Buckinghamshire : Across Services Wycombe General Hospital & Stoke Mandeville Hospital
The Advanced Clinical Practitioner (ACP) will provide high-quality, patient-centred assessment, diagnosis, treatment and management of individuals presenting with a wide range of injuries and illnesses within urgent and unscheduled care settings.
Working autonomously, the ACP will deliver safe and effective care within agreed local, national guidelines, drawing on advanced clinical reasoning and evidence-based practice. The role includes independent prescribing, complex decision-making, and holistic patient management across all age groups, from paediatrics to older adults.
The ACP will work collaboratively within a multidisciplinary urgent care team, contributing to service development, quality improvement, and clinical education.
Working Pattern and Expectations- This is a full-time clinical role, with a minimum commitment of 24 hours per week.
- The urgent care service operates 24 hours a day, 7 days a week, including weekends and bank holidays.
- Clinicians are expected to work days, evenings, nights, and weekends.
- A minimum of one weekend per month is required as part of the rota.
- Shifts are not fixed; flexibility is essential to support safe and effective service coverage.
- Rota schedules are released 12 weeks (3 months) in advance to support worklife balance and planning.
- Requests for flexible working patterns will be considered where possible, in line with service needs.
Clinical Practice
- Assess, diagnose, treat, and discharge or refer patients with minor illness/injury.
- Manage undifferentiated and complex presentations using advanced reasoning.
- Prescribe and administer medicines within scope and protocols.
- Conduct face-to-face and remote consultations for all ages.
- Request and interpret investigations, including ECGs and imaging.
- Work autonomously while collaborating with medical, nursing and AHP teams.
- Provide urgent and emergency interventions as required.
- Maintain accurate and timely clinical records.
- Maintain registration (NMC/HCPC) and uphold professional codes.
- Take accountability for evidence-based, patient-centred decisions.
- Follow all FedBucks and regulatory policies, including safeguarding and confidentiality.
- Engage in reflective practice, supervision, and ongoing CPD.
- Participate in audit, service evaluation and incident review.
- Contribute to pathway and guideline development.
- Promote a culture of safety, quality and compassionate care.
- Support effective communication within the team and with partners.
- Maintain advanced knowledge through CPD.
- Teach, mentor and supervise junior staff where appropriate.
- Support competency development within the urgent care team.
- Engage in service improvement or research projects.
- Complete all mandatory and role-specific training.

Person Specification Experience Essential- 5 years post registration experience of which 2 years will have been spent in primary care, ED, unscheduled care, or related field.
- Experience managing both adult and paediatric presentations.
- Previous experience in GP OOHs, walk-in centre, or MIU.
- Experience in clinical guideline or pathway development.
- Advanced assessment, diagnostic and decision-making skills.
- Excellent communication, documentation and digital literacy.
- Competence in both illness and injury management.
- Knowledge of clinical governance, safeguarding and infection control.
- Ability to work autonomously and collaboratively within an MDT.
- Self motivated, adaptable and professional.
- Flexible to meet service needs, including weekends and evenings.
- Committed to evidence based, patient centred care.
- Project management or research skills.
- Registered Clinician with NMC/HCPC.
- Masters degree in advanced clinical or Nursing Practice (equivalent or higher).
- Minor Injuries and illness qualification.
- Independent prescriber.
- Doctoral level studies.
- Teaching and assessing qualification.
