Advanced Clinical Practitioner

The post holder will provide advanced and expert level of practice (including non-medical prescribing)within the adult Community Mental Health Team leading to improved patient outcomes.
The Advanced Clinical Practitioner (ACP) will act as a professional resource for the multi-disciplinary team(MDT) patients and carers within the scope of their practice and competence, The ACP will understandprofessional accountability and legal frameworks for advanced practice and be professionally accountableand legally responsible for all aspects of own work. The ACP will demonstrate safe practice in line withTrust policies and procedures.
Main duties of the jobThe post holder will support the clinical and operational team to deliver on service improvements in thequality of care and safety through service development, evaluation, and audit.They will be an integral part of the MDT supporting clinical decision making and supporting the servicepathways, developing nurse led clinics and providing brief interventions. The post holder will also supportwith providing a seamless pathway into & out of the community mental health team (CMHT) onceinterventions / goals have been achieved, to promote recovery and self-management, as clinicallyappropriate.The ACP will hold a case load and will lead assessment, formulation and prescribing of care planningincluding discharge planning within agreed protocols and procedures.The post holder may additionally lead on practice and service developments. The post holder is required todemonstrate clinical and practice expertise via interventions, supervision and modelling good practice.To support improving workforce knowledge and practice through the contribution to a CMHT education andtraining programme across the community teams within BoltonTo participate in the Bronze on site / Bronze on Call rotaThe core hours are Monday to Friday 9-5, although some flexibility, including weekend / BH working may be required

- Extensive knowledge of psychological, social and biological factors and formulating these into a comprehensive bio-psychosocial formulation and clinical risk management plans.
- Clinical supervision training and experience
- Experience of coordinating & leading on a series of audits, action plans & projects
- Extensive knowledge and application in practice of leading complex adult safeguarding procedures, professional meetings, MHA, CPA, DOLs
- Experience of delivering education and training to clinical staff eg clinical risk training / physical health education
- Knowledge of issues affecting the client group
- Experience of leading / supporting to complete investigations & Rapid Reviews of Care & complaints
- Communication skills, both written and verbal
- Communication of difficult messages to service users and families
- Negotiation and conflict management skills
- to be aware and keep up to date with of the body of knowledge related to practice in a specialist community mental health team setting.
- Understand and comply with Trust standards and policies
- Understand the Trusts strategic goals and how they can support them.
- Understand the need to safeguard children and adults.
- Carry out all duties and responsibilities of the post in accordance with equality and diversity and dignity in care and work policies and principles.
- Maintain high standards of quality in corporate and clinical record keeping ensuring information is always recorded accurately, appropriately and kept up to date.

- Engagement in and delivery of supervision
