Advanced Clinical Practitioner / Advanced Nurse Practitioner

We are looking for an experienced Advanced Nurse/Care Practitionereducated to masters level in Advanced Health Assessment, holding a Prescribingqualification.

Working as part of our urgent care team, you will work autonomously yetcollaboratively to provide advice, diagnosis and treatment both on the phoneand face to face in surgery or within the community. Part of the role includesthe review of patients with a Nursing Home setting, therefore you will need theuse of your own car.

Main duties of the job

Provide the choice of direct access to a ANP/ACP, both in the practiceand over the telephone, and dependent on the individual, at home and in thecommunity for the general practice population with an acute or chronic healthproblem

Provide a first point of contact within the practice for patientspresenting with undifferentiated, undiagnosed problems, making use of skills inhistory taking, physical examination, problem-solving and clinicaldecision-making, to establish a diagnosis and management plan

To prescribe safe, effective and appropriate medication as defined bycurrent legislative framework. Registered Independent Prescribing.

Offer a holistic service to patients and their families, developingwhere appropriate an on-going plan of care/support, with an emphasis onprevention and self-care.

Promote health and well-being through the use of health promotion,health education, screening and therapeutic communication skills.

Work with multi-disciplinary team within the Practice and with otherservice providers to promote integrated and seamless pathways of care

Assess acute and chronic illness in the surgery and in Nursing and Carehomes.

Job responsibilities

The post holder should be anexperienced nurse or paramedic, educated to at least Masters Level in advancedhealth assessment and nonmedical prescribing who will act within theirprofessional boundaries to provide care for the presenting patient from initialhistory taking, clinical assessment, diagnosis, treatment and evaluation ofcare. They will demonstrate safe, clinical decision making and expert care forpatients within the surgery.

The Practitioner will have theability and training to work autonomously yet collaboratively within the team toprovide clinical care and support colleagues within the team clinically andoperationally.

The role includes additionalduties remunerated dependent upon provision of evidence and indirectobservation of duties.

Qualifications
  • RGN current NMC registration with minimum 4 years post registration experience
