Adult's Social Worker, CHC team, Lancashire Pay rate £33 per hour Contract role, Continuing Health Care Pertemps are recruiting for an experienced Adult's Social Worker in Lancashire This is to work in a Continuing Health Care team in the Fylde and Wyre area. The role

• In this Social Work role you will complete Care Act assessments and participate in Multi-Disciplinary meetings to consider eligibility for Continuing Health Care for individuals who have been discharged from hospital with Discharge Support funding. These individuals will typically be in a bed based setting.

• You will work to defined timescales and promote interventions based upon the client's strengths and focus on what is important to them, offering a truly holistic strength based approach. You will have the ability to engage with service users, their carers and system partners to undertake these assessments virtually and to commission appropriate care to promote independence and meet identified needs.

• The post on offer is a permanent, 37 hour contract working Monday to Friday.

• We are looking for an enthusiastic Social Workers with a passion for promoting independence and supporting the Discharge to Assess model.

• You will need to be flexible and adaptable in a constantly changing environment as well as having a commitment to improve the lives of our service users through creative, evidence based practice.
