Adult's Social Worker

We are currently recruiting for an Adult's Social Worker to join Team.

This role requires a Social Work Qualification with a minimum of 2 years post qualified experience.

About the team

team specialises in providing an urgent response service to prevent service users being admitted to hospital. The team works to provide rehab and reablement to service users within the community to ensure quality of life. This is an integrated service with Adult's Social Care and the NHS.

About you

Experience within an older people physical disabilities, MDT or hospital based teams lends well to the type of work this team deals with. A degree within Social Work (Degree/DipSW/CQSW) with a minimum of two years experience working in an Adult's Social Work setting are essential for this position.
