Weare looking for a dynamic individual to meet the demands of this fellowshippost in a high profile area of complex and demanding surgery.

The Department of Neurosciences in Leeds providesWorld-Class Spinal services within an integrated Neurosurgical and OrthopaedicSpinal department. The service is consultant-led and delivered throughdedicated sub-specialty services. The fellowship has approval from the RoyalCollege of Surgeons of England and is designed for Neurosurgical clinicianstowards the end of training and will provide all aspects of sub-specialtytraining in complex spinal surgery in preparation for a consultant rolesub-specializing in this field.

Main duties of the job The Neurosurgical Spinal post will include athorough grounding in the clinical management, investigation and surgicaltechniques for optimally managing adult patients with all forms of spinalsurgical disease. Leeds General Infirmary operates a strong MDT approach tospinal disease with close and collaborative links between specialistneurosurgical and orthopaedic spinal consultant teams. There are significantfurther opportunities to develop research, and work with researchers and theclinical nurse specialist team is expected. The fellow will gain independentcompetencies in dealing with adult patients presenting with acute and electivespinal disease including cranio-cervical pathology, spinal cord and intraduraltumour pathology and multilevel congenital and degenerative spinal disease.

INFORMATIONABOUT THE DEPARTMENT

TheDepartments of Neurosurgery and the Integrated Spinal Service provide acomprehensive neurosurgical and spinal surgery service to the 2.5 millionpeople of West and North Yorkshire.The Departments are housed within the Jubilee Wing of the GeneralInfirmary at Leeds.

TheDepartments have 68 adult beds in two dedicated wards with an additional 7bedded dedicated Neuro-intensive Care Unit as well as a 7 bedded dedicatedHigh Dependency Unit. There are fouradult operating theatres including one dedicated to emergency surgery 24hours per day. There are separatebeds, operating theatres, and intensive care facilities for PaediatricNeurosurgery. The integrated spinal service offers all aspects of spinalsurgery and is made up of 4 Neurosurgical and 8 Spinal Orthopaedic Consultants.

Thesurgical services within the CSU have close links with the other neurosciencespecialties that are all on site including Neuroradiology, Neurology,Neuroanaesthetists and Neurophysiology.The Neuro-pathology department for Yorkshire is now based at StJamess hospital but continues to provide a full histological and post-mortemservice including facilities for inter-operative histopathology.

TheNeuro Radiology Department is based at the Leeds General Infirmary andcomprises 7 Consultants, three of whom provide Endovascular services.

TheGeneral Infirmary at Leeds is a level 1 trauma unit (MTC status) and thereare close links with other associated surgical specialties including trauma,maxillofacial, plastics, cardiothoracic and ENT surgery.

Thedepartment's work has continued to increase over the past years. The current caseload is some 3000inpatients per year and approximately 2000 operative cases per year.

TheCentre for Neurosciences in Leeds provides world class adult Neurosurgicaland Orthopaedic spinal care. Theservice is consultant led and delivered through dedicated subspecialtyservices. The spinal services havevery strong links with Neurology, Neuroradiology, Neuropathology, and LeedsUniversity.

Aswould be expected, to support spinal surgery, all the theatres have BrainLABneuronavigation, CUSA, Sonopet, Zeiss microscopes (including navigated),X-ray support, intra-operative frozen section, and intra-operativeultrasound. All patients are discussedin a weekly MDT that conforms to the BASS standards and all patients areentered into the National Spinal Database.

TheNeurosurgical Spine consultants (Mr Pal, Mr Derham, Mr Selvanathan, and MrSinha, and Mr Kellet) integrate with the Spinal Orthopaedic consultant team(Mr Gummerson, Mr Rao, Mr Khan, Mr Rudol, Mr Loughenbury, Mr Borse, and MrDunsmuir) and run a combined on-call rota. There is a designated specialistCervical Spine MDT clinic which runs on a weekly basis. All patientsundergoing operative intervention are discussed at a weekly Neurosurgicalspine and Spinal MDT. All aspects of Spinal surgery are offered within thedepartment. The department currently supports 3 fellowship posts in spinalsurgery including the Stryker Industry Fellowship and the National SpinalFellowship. This new fellowship post is intended to allow an experiencedtrainee, from a Neurosurgical background and peri-CCT the opportunity to gainmore experience over a one-year period in this sub-specialty within spinalsurgery with a specific interest in craniocervical and intramedullarymicrosurgery. It is expected that sufficient experience will be obtained fromthis fellowship to allow a successful fellow to offer themselves forappointment to a consultant position involving a substantial commitment tothis specialist area of surgery. It is expected that through the fellowshipprogram, experience is gained in the assessment, investigation and operativemanagement of patients presenting with both acute and elective complex spinalpathology. It is also expected that the Fellow will gain experience in themanagement of degenerative, neoplastic, infective, and traumatic disorders ofthe spine. The Fellow will be expected to perform most of the complex spinalcases (depending on their level of experience) and will be trained intechniques including complex cranio-cervical junction and sub-axial spinereconstruction, microsurgical techniques in relation to the spinal cord,minimally invasive techniques (MIS, Vertebroplasty, XLIF), and spinal fusiontechniques. Participation in a weekly Spinal MDT consisting of Neurosurgeonsand Spinal Orthopaedic surgeons, Radiologists, and specialist nurses will beexpected. The Fellow will be encouraged to participate and conduct researchinto complex spinal disorders and attend national and international meetings.The Fellow will be expected to participate in the general neurosurgicalregistrar on call rota, to provide continuity of care for patients, andmaintain a general emergency skill set in neurosurgery. As such, preferencewill be given to candidates applying with base specialty skills inNeurosurgery or FRCS (SN). It is expected that the Fellow will shareresponsibility for the neurosurgical patients in their area of sub-specialistinterest outside of strict rota commitments.

Thereare very strong links with Leeds University and local brain tumourcharities. There is a dedicated SpinalResearch Nurse.

Person Specification Qualifications

• MBBS or equivalent medical qualification
• ARCP Outcome 1 at ST6 or equivalent experience
• Passed Speciality Fellowship Examination in Neurosurgery or equivalent
• CCT in Neurosurgery
