Calderdale Council continue to develop services which support young people to access support if they are threatened with or present as homeless. The Young Persons Housing Team supports young people 16/17 yrs, families, and agencies with interventions to prevent the need for any young person requiring emergency homeless accommodation. As part of our wider prevention strategy, we are looking to appoint a young person's homelessness prevention worker to strengthen our existing provision.
This role will support the Young Persons Housing Team in conducting assessments, providing support to young people and their families and to engage with partner agencies to reduce the numbers of young people requiring emergency homeless accommodation.
You will primarily work with 16- and 17-year-olds and 18- to 25-year-old Calderdale care leavers. Your duties will include holding daily responsibility for the identification and homelessness assessment of young people, visiting young people and families at home, providing prevention interventions, exploring planned supported accommodation pathways, supporting young people to access accommodation, carry a caseload of young people, support partner agencies in implementing and sharing knowledge around youth homelessness, plan and arrange meetings between all parties, provide support to young people with a variety of interventions in areas such as budgeting, access to welfare benefits, accessing health services and linking in with other young people's services.
