Trainee Advanced Clinical Practitioner - Older Adult Mental Health

We are looking for a Trainee Advanced Clinical Practitioner to join our Community Mental Health Team in the Wyre Forest.

The team consists of Mental Health Nurses, Occupational Therapists, Psychologists, Support Workers, Junior Doctors and Consultant Psychiatrists. Your role will be to support the diagnostic process in our Dementia pathway and support with the challenges experienced by those living with Dementia and their families/carers. You will also work across our Mental Health Needs pathway, assessing, supporting and diagnosing patients with functional mental health conditions.

You will need to have knowledge and experience in Older Adult Mental Health. This role includes liaison work, education, and clinical expertise in recognising and managing Mental Health needs from both a pharmacological and non pharmacological perspective. Advice on management of behavioural and emotional expression of unmet need experienced in Dementia will form a large part of your role. Developing a good working relationship with the wider MDT and our partners will be essential.

You will be part of a supportive and friendly team and supervised by a Consultant Psychiatrist. We would expect a successful applicant to be able to start the university course in January 2026.

The core working hours for our team are Mon Friday (9 am - 5 pm) and this is a full time role.

Main duties of the job

Undertake a full biopsychosocial assessment of patients including full history, taking, determining a differential diagnosis and initiating initial management including appropriate referrals. This care and assessment will be undertaken under the direct or indirect supervision of the medical team/qualified ACP.

Demonstrate in depth knowledge and capabilities in all aspects of health and social care encompassing advanced clinical assessment, critical thinking and clinical management skills that are evidence based and deliver high quality patient centred care.

Prescribe medication within the scope of their discipline and experience in the case of this role, older adult mental health.

Work with other Healthcare professionals to prioritise healthcare needs and intervene appropriately to assist the patient in complex or urgent care situations and escape as needed.

Participate in MDT discussion and establish effective lines of communication across services, teams and agencies to ensure contemporaneous feedback on treatment and interventions and facilitate high quality, safe, effective care.

Job responsibilities

Develop, implement and evaluate individual plans of care with patients and their carers/relatives according to their current healthcare needs and differential diagnosis to enable step up or step down to appropriate services when their condition requires.

Integrate both drug (where within scope of professional practice) and non drug based treatment methods into a care management plan, with the support of appropriate practitioners, and provide a range of treatments using patient group directives or non medical prescribing, following clinical assessment and diagnosis.

Monitor and evaluate clinical outcomes using a patient centred approach, involving the patient and their carers where appropriate.

Take every opportunity to promote the health, wellbeing and independence of the patients and initiate health education and self care strategies as appropriate.

Work within appropriate legislation, policies and best practice evidence relevant to clinical area.

Initiate and review ReSPECT documents as required with patients and where appropriate their carers/relatives.

Qualifications
  • Registered Nurse Level 1/Allied Health Professional/Paramedic/Pharmacist
  • Undertaking MSc in Advanced Clinical Practice
  • Non Medical Prescribing (where within scope of professional practice). Where not within scope of practice they must have completed an equivalent number of credits in another relevant masters level module from the ACP masters
  • Advanced Assessment Skills Module
  • Evidence of a portfolio of knowledge, skills and professional development
