Job Description
St Margaret's Hospice Care has a fantastic opportunity for a Specialty Doctor to join our team.

This role is based in the Taunton Inpatient Unit with 1 in 8 weekends on-call, and a one night per week covering Taunton IPU. You will join us on apermanent basis, working 16 hours per week (4 sessions).

Pay will be basedon level of experience using the NHS speciality Doctor pay scale.

Working as part of a multi-disciplinary team, you will be based at St Margaret's Inpatient Unit in Taunton. There are 12 beds in the unit, used for patients who need specialist symptom control or have complex end of life care needs.

IT training and induction will be provided prior to commencement of the post, with access to ongoing support via the wider hospice education programme.

Main duties of the job Your day-to-day work will involve:
  • Providing specialist medical care, advice and support in order to provide high quality individualised care to patients/clients in line with local and national guidelines.
  • Providing medical assessment and care for all patients on the inpatient unit, in line with the care plan ensuring that medical care is evidence-based (as far as is possible) and best practice.
  • Holistically assess patients admitted to the inpatient unit, planning initial management in discussion with the available senior doctor as appropriate.
  • Completing holistic assessment records, discharge letters, letters notifying death, TTO prescriptions and anticipatory prescribing.
The weekend role involves a ward round of the inpatient unit each Saturday and Sunday following handover with the nursing team. The on-call is then for phone advice as required but there may be a need to revisit the unit in the case of an emergency admission or to review a patient acutely. Second on-call phone support is provided by a Palliative Care Consultant.

Job responsibilities Clinical:

  • Providemedical assessment and care for all patients on the inpatient unit, in linewith the patients care plan, as part of the multidisciplinary teamensuring that medical care is evidence-based (as far as is possible) and bestpractice.
  • Worktogether with other members of the medical team to ensure each patient isreviewed as frequently as their symptom management dictates (usually at leastonce per day).
  • Participationin weekly inpatient MDT meeting if working hours include this.
  • Holistically assess patients admitted to theinpatient unit, planning initial management in discussion with the availablesenior doctor as appropriate.
  • Carry out basicclinical procedures as necessary including phlebotomy and cannulation asnecessary. The post holder will not be expected to attempt procedures inwhich they do not feel competent.
  • Maintainaccurate and timely patient records. Medical records are electronic based (Systmone).
  • Ensurelegible prescribing in accordance with local policy.
  • Liaise withprimary and secondary care services (and other statutory and voluntaryagencies) as appropriate to ensure continuity of medical care.
  • Complete holistic assessment records (including drugreconciliation charts, VTE screening), discharge letters, letters notifyingdeath, TTO prescriptions and anticipatory prescribing in line with localguidance. (As appropriate within the hours worked.)
  • Confirmation and certification of death in line withhospice protocol, liaising with the Medical examiner service and CoronersOffice where required by law and local practice.
  • Liaise with family members/carers as appropriate.

Out-of-hours,maintaining principles as above, additionally:
  • Undertakeemergency admissions, in agreement with the out-of-hours consultant
  • Re-assess,review and adjust treatment of any inpatient where necessary due to change inpatient condition
Teaching and Training:
  • Participate inthe induction for new members of staff.
  • To take theopportunity to take part in in-house teaching and training if appropriate.
ClinicalGovernance:
  • To always adhere to the GMC code of professionalconduct.
  • To work within St Margarets Hospice organisationalpolicies and to implement policies within area of responsibility.
  • To work withother members of the team to ensure that evidence from research issystematically considered and practices changed as appropriate.
  • To activelycontribute to audits and review of service if hours allow.

Other responsibilities
  • To work within Hospice policies and procedures toensure safe and effective working practices for self and others (e.g. Healthand Safety, COSHH, emergency procedures, the Mental Health Act, ChildProtection procedures)
  • Toalways maintain patient confidentiality within Hospice policy and procedures.
Person Specification Education and Qualifications
  • Full registration and licence with the GMC.
  • MB, BS, or equivalent
  • Completed at least 3 years full time post registration training (or its equivalent on a part time or flexible basis) within a relevant training program such as GP training, general medicine or oncology.
  • Experience of assessment and management of patients with life limiting conditions.
  • Diploma in Palliative Medicine or equivalent.
  • MRCP/MRCGP/FRCR
  • At least 1 years experience in Palliative Medicine.
  • Experience in working in Specialist Palliative Care inpatient services.
  • To demonstrate an understanding of evidence based medicine.
  • To have a basic knowledge of the management of symptom control, including familiarity with prescribing opioids.
  • To have an understanding of the roles and contributions of other members of the Specialist Palliative Care multidisciplinary team.
