Main duties of the jobThe Frailty Directorate is expanding with new services such as a 7-day Same-Day Emergency Care and Rapid Access Care of the Elderly clinics. This role supports the development of an Integrated Frailty Medicines Optimisation Pharmacy Network.
Our team focuses on continuous improvement for patient benefit, welcoming new ideas. We aim to upskill our workforce, including pharmacy assistants supporting ward medicines management, dispensing, and discharge checks to enhance safety and reduce waste.

Job responsibilitiesAs part of a multidisciplinary team within the frailty service, you will provide a 7-day patient-facing clinical service supporting medicines optimisation, including participation in research.
You will ensure the safe and effective use of medicines to improve outcomes, reduce polypharmacy, and enhance patient understanding and adherence.
Involvement will include ward rounds, clinics, and collaboration with Primary Care Networks and Medicines Optimisation teams. The team also provides training, conducts audits, and contributes to service development and planning.
You will engage in service development projects, focusing on safe medication transfer and interface care, improving patient flow and care quality.
Person Specification Qualifications- Master's degree in Pharmacy and successful pre-registration training
- Registered with the General Pharmaceutical Council (GPhC) or eligible
- Postgraduate Diploma or equivalent (or working towards it)
- Understanding of frailty syndromes and medicines optimisation principles
- Strong core therapeutic knowledge
- Hospital pharmacy experience
- Good ward practice, medicines management, ward rounds participation
- Provide an example of a role model you have acted as
This role is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975. A DBS check is required to verify any criminal convictions.
Salary: £53,755 to £60,504 per annum/pro-rata
