Job overviewThe Specialist Occupational Therapist (OT) functions as part of the Multi-Disciplinary Team (MDT) to provide a seamless service to a group of patients.
The OT will develop, implement, and evaluate the Merton Enhanced Home-Based Rehabilitation Team (MEHBRT), ensuring that patients receive the highest standard of timely care within the trust
Main duties of the jobThe Specialist Occupational Therapist (OT) functions as part of the Multi-Disciplinary Team (MDT) to provide a seamless service to a group of patients.
The OT will develop, implement, and evaluate the Merton Enhanced Home-Based Rehabilitation Team (MEHBRT), ensuring that patients receive the highest standard of timely care within the trust.
The OT will clinically assess referrals from hospitals for patients who are ready to be discharged but have ongoing health or social care needs.
The OT will provide local clinical leadership and advice to Physiotherapists, Rehabilitation Support Worker and the wider MDT within the CSU, while also being responsible for the education and supervision of rehabilitation support workers, and MDT students.
The OT will be responsible for the line management and supervision of junior staff within the service.
The OT will manage their own caseload, including complex cases and request support from Senior members if needed.
The OT will lead/support the team in achieving key performance indicators (KPIs) and maintaining the quality of the service.

Person specification Qualifications- Degree or diploma in Occupational Therapy
- Current AHP with UK HCPC registration
- Up to date evidence & relevant knowledge through CPD as evidenced by portfolio
- Membership of Royal College of Occupational Therapy and / or relevant special interest groups
- Attendance at Clinical Educator Course
- Appropriate post-qualifying experience in a range of areas acute care of the elderly, neurology and community
- Appreciation of constraints to working in the community
- A clear understanding of current NHS issues, professional standards, CPD, clinical governance and experience of audit
- Ability to assess patient conditions, establish the patients problem, formulate and implement a treatment plan
- Has sound knowledge of the Mental Health Act and other legislation
- Experience of MDT working and understand their roles
- Some experience of supervising juniors/assistant/students
- Some experience in research / literature reviews
- Clinical Teaching
- Experience in community work within rehab
- Post registration experience at band 5 level in a wide range of clinical areas including elderly rehabilitation, community rehabilitation, falls management, dementia care, respiratory care and orthopaedics
- Some understanding/experience in quality improvement and service evaluation.
- Using NHS clinical systems
- Experience in teaching and training
- Experience of line managing / Supervision of staff
- Knowledge and understanding of National and local frameworks and guidelines for older people.
- Knowledge of outcome measures and evidence based practice associated with people with physical disabilities
- Advanced communication and report writing skills, in both verbal and written format
- Highly developed specialist knowledge and application of assessments and interventions relevant to client group
- Understanding and application of occupational therapy practice outcome measures
- Hold a full, valid, UK driving licence and have access to a car to use for business purposes (unless you have a disability as defined by the Equality Act 2010)
