The Memory Assessment Service is an NHS service provided by Here inpartnership with Alzheimers Society, Sussex Partnership NHS Trust and theBrighton and Hove Carers Centre. Theservice was launched in June 2013 and is run by Specialist GPs, Nurses andMemory Support Workers. MAS provides access to memory testing and diagnosis,and offers advice, care and support for people with dementia and their caregivers.

Our service function isto assess, diagnosis and provide support and care planning for people withdementia to enable them to lead fulfilling lives while future planning.
The role of the SpecialistDementia Assessment Clinician is to operate as part of the clinical team to undertakespecialist cognitive assessment and contribute to diagnosis formulation. You must be a registered clinician (e.g., anurse, occupational therapist, psychologist, neuropsychologist, etc). withspecialist training and experience in dementia assessment and care.

The post holder will bean autonomous practitioner who will carry responsibility for our patients byundertaking the following duties:
1.Provideassessment using standardized psychometric testing (ACE), clinical interviewand reviewing wider history and notes.
2.Deliverassessments in a variety of settings, including clinic, virtual and home visitsif required.
3.Discussand review cases with MDT to confirm and diagnosis dementia where present.
4.Deliverdiagnostic information to patient, providing information about support servicesand treatment options that may be available to them.
5.Ensurethat clinical practice is evidence-based and consistent with relevant NICEguidelines.
6.Ahereto relevant policies and procedures for the Here and MAS.
7.Attend(virtual) MDTs or supervision to contribute to diagnosis formulation.
8.Beaware of treatment and support options available.
9.Helpcarers understand the value of engaging with support services.
10.Workin a collaborative way, ensuring the best care for our patients.
11.Ensurethat any transfer of care is seamless and integrated across system pathways.
12.Ensurethat up to date written and electronic records and activity data are maintainedin accordance with professional and organisational /service standards.

14.Providesupervision and training of junior staff and trainees where appropriate.

Person Specification Qualifications- Registered healthcare professional with specialist training appropriate to the job role and/or equivalent experience.
- Completion of Mentorship Course/ENB equivalent
- Evidence of continuous professional development
- Skills of assessing and interpreting service user conditions with appropriate actions
- Ability to use manual handling techniques
- Assessment, planning and intervention skills.
- Provision of specialist advice in relation to care
- Excellent written and oral communication skills
- Ability to communicate in a clear and unambiguous way

- Ability to develop good therapeutic relationships

- This role requires travel to different sites.
