Social Assistant at 70% multi-sites

Deployed across multiple sites, the Institution of Lavigny is 70% multisite. The Institution of Lavigny supports adults with disabilities and/or neurological disorders through a comprehensive, interdisciplinary approach centered on the person. The institution includes: a hospital, one of the reference centers in French-speaking Switzerland for brain injuries requiring neurological care, or suffering from difficult epilepsy; a living and care site, Plein Soleil in Lausanne, offering accommodation (EMS), socio-cultural activities, and individualized support for adults with neurological diseases or brain injuries; and operates with a logic of continuity of care pathways and strengthening the link between medical, social, and institutional dimensions. We are looking for a: 70% multisite position. Activity split 50% at the Hospital of Lavigny and 20% at Plein Soleil in Lausanne. Your mission: Within multidisciplinary teams, you will act as the social reference for social questions, closely linked with the patients' families and partner organizations. Your main responsibilities include: addressing social needs and securing the needs of hospitalized patients and residents; assisting with financial and social processes while considering vulnerabilities related to neurological disorders; working with families and external networks (insurance companies, social services, partner institutions); actively contributing to interdisciplinary reflections and discussions to support the coherence of support projects; and contributing to the continuity of care pathways between returning home, institutional care, or maintaining a living environment. A social resource role for the teams, in a supportive and respectful manner towards frameworks.
