Gateway Housing Solutions

Apsley, Hemel Hempstead

24 hours per week

the Housing Solutions team offer specialist assessment across Hertfordshire for major housing adaptations. The team has a blend of community based Occupational Therapists and Community Care Officer Trusted Assessors who complete specialist housing needs assessments to identify major adaptation solutions to enable a person to have better access to the facilities within their homes.

We explore personal strengths, their family and friends' support networks, and using expert knowledge we connect people with things that will help individuals and families to get on with their lives, achieving prevention, reducing or delaying the need for statutory provision of care services.

Older Peoples West - Review Team

Apsley, Hemel Hempstead

37 hours per week

We are seeking a skilled and motivated Senior Occupational Therapist to join our Community Review Team. Community Review Team focuses on reviewing care and support arrangements for adults in the community, ensuring that care remains appropriate, proportionate, outcome-focused, and promotes independence. We also work alongside Residential Review Team and there is a scope for joint working/ working across the teams.

The Community Review Team undertakes planned reviews using a strength-based, outcome-focused approach. Wherever possible, the team identifies opportunities to connect individuals with appropriate community resources. Reviews typically cover home care, day care, Direct Payments/Individual Service Funds, Carers Direct Payments, and Flexicare.

Older Peoples West - Harpenden and Villages

The Older Person Service is a dynamic service that works across the community under the Care Act to safeguard, assess, plan support and review adults and cares with long term health and social care needs. We can offer Hybrid working arrangements, flexible working practices, community engagement and volunteer activities and, as part of our commitment to you, as well as promote your continuous Learning & Development.

Older Peoples East - Welwyn Hatfield and Broxbourne Team

IDS West

Post Hospital Review Team

37 hours per week

Our Integrated Discharge service in the West of Hertfordshire supports an integrated approach to managing the care of people who require support through the admission and discharge pathway, so the role will involve working alongside Occupational Therapists, Community Care Officers, health colleagues and care providers. You will have the ability to undertake innovative assessments and care planning with individuals, carers and their personal support networks.

We promote personalisation focusing on the wellbeing of the individual. Through care planning we strive for increased independence, choice and control by enabling people to direct their own support. You will gain extensive experience in the field of adult social care, safeguarding practice and multi-disciplinary working.

Adult Disability Team East - Stevenage

37 hours per week

Fixed Term or secondment contract to January

Providing support to adults living with a learning or physical disability.

We aim to enable people to make positive decisions in their lives, promote their independence and ensure they can get involved in their local community. We're a service motivated by and dedicated to social care intervention and community social work.

We work in an enabling way with people and their families and carers, to promote independence and participation, through meaningful occupation and a strengths-based approach. Across all our services we aim to ensure that people are provided with the right support and advice, and at the right time.

About the role

  • Working collaboratively with other partners and teams within Adult Care Services
  • Developing innovative support plans
  • Promote and support strength based assessment and practice approach to all interventions to maximise independence, health and well being to prevent or reduce the need for Adult Social Care
  • Working creatively with residents to ensure their safety and independence
  • Raising awareness in the area and developing valuable links for the team
  • Supporting other Therapists and Community Care Officers
  • Provide information and advice to enable an individual to confidently and safely participate in their daily life, includes grading of tasks and activities and adapting their environments
  • You will undertake a specialism eg Moving and Handling, BiA, AMP or Occupational Therapy practice educator

About you

Essential:

  • HCPC Registered Occupational Therapist
  • Full driving licence and regular access to a vehicle
  • 2 years post qualifying experience as an Occupational Therapist
  • Knowledge and experience in social care sector
  • Experience of completing assessments and making recommendations
  • Ability to work on your own as well as part of a team

Desirable

  • Knowledge of Care Act 2014
  • Knowledge of Mental Capacity Act 2005
