Senior Geriatrician: Frailty & Hospital-at-Home Lead

County Durham & Darlington NHS Foundation Trust is seeking a Consultant Geriatrician to lead frailty care across hospital and home-based pathways, including and Frailty SDEC. The post collaborates with MDTs to deliver CGA and rapid decision making.

The successful candidate will oversee clinical governance, develop evidence-based protocols, supervise junior doctors, and contribute to NHS 10-Year Plan objectives, with a 1:7 on-call roster.
