The EBBI Neighbourhood Team is a friendly, fun loving, flexible, and hardworking team who are looking for an enthusiastic individual to join us at our base in Evesham. This is a part time post working 34 hours per week.
The Neighbourhood Team is looking to recruit a senior community nurse who is happy to work across our whole patch, who is innovative and keen to join the team in supporting us to deliver high quality care, and to continue our development of integrated care in the community.
You should be excited by change and keen to help shape a new way of delivering nursing care in the community. We have a team of creative health care professionals who are focused on compassionate care. We work closely with our GPs and work hard at developing relationships with other primary care services, drawing on the skills of our MDT colleagues to manage complex patients and ensure our services are as responsive and patient centred as possible.
We are a supportive team who are committed to developing services to provide the most individual care for our patients. We offer dedicated time for training and clinical supervision.

Main duties of the job- The ability to communicate effectively with others at all levels
- Working flexibly to provide the service delivery need across the local area (the service operates from 7:30am to 6:30pm, Monday to Sunday including bank holidays)
- An enthusiastic, motivated, and innovative thinker to become part of a primary care facing community
- The ability to work autonomously and manage your own case load whilst supervising others and working within a multi disciplinary team

Work as an autonomous practitioner with the integrated care team providing rapid and planned expert professional assessment and treatment interventions to patients in their own homes or other community settings to enable patients to maximise their independence, health and wellbeing and remain in their own homes for as long as possible. This will be achieved through a combination of hands on expert clinical skills and knowledge to treat, educate and support patients on a designated caseload and by working with patients and their carers to assess, plan, implement and evaluate their ongoing care and support needs in their own home or temporary/usual place of residence.
Demonstrate expert clinical knowledge, health promotion and holistic assessment skills and will work with the patient and their carers to assess, plan, implement and evaluate their ongoing care and support needs in their own home or temporary/usual place of residence.
Support the Operational and Clinical Team leaders in the delivery of best nursing practice, be involved in triage, allocation of work and leadership of other staff, manage team resources and deputise in their absence.

Person Specification Qualifications- Registered Nurse (RN1)
- Specialist practitioner District Nursing (degree / diploma) or equivalent level nursing qualification
- Non medical prescriber (V150) or will be required to undertake this qualification on commencement in post
- Evidence of on-going continuing professional development
- I understand that I will be asked to provide original certificates to verify my qualifications at point of offer.
