We are looking to recruit a Senior Community Nursewho is happy to work full-time, manage a caseload and who is innovative and keen to support usto deliver high quality care, and to continue our development of integratedcare in the community.
We are looking for a passionate individual who isexcited by change and is keen to deliver nursing care in the community. We havea team of creative healthcare professionals who are focused on compassionatecare. We work closely with our GPs and work hard at developing relationshipswith other primary care services, drawing on the skills of our MDT colleaguesto manage complex patients and ensure our services are as responsive andpatient centred as possible.
We are a supportive team who are committed todeveloping services to provide individual care for our patients. We offerdedicated time for training and clinical supervision, with a supportive L&DDepartment to help guide.

Main duties of the jobWorkingflexibly to provide the service delivery need across the local area (theservice operates from 7:30am 6:30pm, Monday to Sunday including bankholidays).
Anenthusiastic, motivated and innovative thinker to become part of a primary carefacing community.
Theability to work autonomously and manage your own case load and work within amulti-disciplinary team.
Experienceof working in a fast paced and varied environment.
Your typical working day will include homevisits for adults needing a range of nursing interventions. Theseinclude caring for people reaching the end of their life, managing their painrelief and other symptoms in a sensitive and compassionate manner, all types ofwound care- post operative wounds, leg ulcers, pressure ulcers and complexdressing regimes - you will become expert in tissue viability. Other skillsrequired will be in IV therapy and other injectable drug administration and holisticassessment of a persons needs, taking in to account their environment andsocial circumstances.
Back at your base, you will be a caseloadholder supporting a team of junior staff with delivering individualised careplans, you will be involved in team meetings, goal/care planning and evaluationand handover meetings. You will also support the teaching and learning of nursestudents pre and post registration. Your input is a valuable part of theteams work.

This will be achieved through acombination of hands on expert clinical skills and knowledge to treat, educateand support patients on a designated caseload and by working with patients andtheir carers to assess, plan and implement appropriate packages of care.
Demonstrate expert clinical knowledge, health promotion andholistic assessment skills and will work with the patient and their carers toassess, plan, implement and evaluate their ongoing care and support needs intheir own home or temporary/usual place of residence.
Support the Operational and Clinical Team leaders in the deliveryof best nursing practice, be involved in triage, allocation of work andleadership of other staff, manage team resources and deputise in their absence.

Person Specification Qualifications- Registered Nurse (RN1)
- Graduate Nursing Qualification at Academic Level 6 or working towards.
- Preceptorship training
- Evidence of on-going continuing professional development
- I understand that I will be asked to provide original certificates to verify my qualifications at point of offer.
- Prescribing Qualification or willingness to undertake
- Post-graduate modules in Leadership
- Significant post registration experience in a clinical setting able to demonstrate transferability of experience to community setting
- Experience of management of caseloads or wards and organisation of staff
- Clinical audit/service improvement and development work
- Experience of active participation in interdisciplinary team working
- Transcribing medication training
- Clinical audit/service improvement and development work
- High level of clinical knowledge and skill
