Job Description

As part of our ongoing development and expansion, we are seeking a highly motivated Senior Prescribing Pharmacist to join our dynamic pharmacy team. This is a fantastic opportunity to shape the future of primary care in a supportive and collaborative environment.

The post holder will work within a multi-professional environment, in our pioneering same-day Triage Hub, supporting managing same-day demand, with supervision provided by a Lead GP. This hybrid patient-facing role also involves managing long-term conditions, conducting medication review clinics, supporting clinical medicines management, and contributing to pathway development to enhance our award-winning practice.

Main duties of the job

Key Responsibilities

  1. Same-Day Demand Management: Work within our Triage Hub to assess and manage same-day patient requests, ensuring safe and effective prescribing. Use clinical expertise to support and streamline workload. Provide administrative support in managing related demand.
  2. Structured Medication Reviews: Attend and refer patients to multidisciplinary case conferences.
  3. Long-Term Condition Management: Manage your caseload, run long-term condition clinics (e.g., hypertension, diabetes, asthma, COPD), optimize medication in line with guidance, and ensure appropriate follow-up. Collaborate with the wider clinical team to provide proactive, patient-centered care. Manage risk stratification tools like Eclipse Live, Ardens, and other digital tools.
  4. Repeat Prescribing: Manage the reauthorization process for repeat prescriptions. Support the practice's vision of a fully managed pharmacist service delivery for repeat prescribing. Sign prescriptions within your professional scope.
  5. Transfer of Care: Reconcile medicines from letters and other organizations. Clinically assess new medications for appropriateness and arrange follow-up as needed.
  6. Medicines Management and Quality Improvement: Identify prescribing areas needing improvement, conduct clinical audits, lead suggested changes, and manage the implementation process.
  7. Population and Public Health: Design and manage public health campaigns within the practice.
  8. Clinical Leadership & Innovation: Lead to ensure compliance with CQC standards, mentor junior pharmacists and support staff, lead local prescribing quality schemes, and develop protocols and pathways. Provide training to the in-house team.

Person Specification Qualifications
  • Holds an independent prescribing qualification
  • Able to plan, review, and manage pharmaceutical care, including LTCs
  • Accountable for professional expertise and direct care
  • Recognizes clinical priorities and refers appropriately
  • Adheres to legal, ethical, and GPhC standards
Experience
  • Minimum of 3 years' experience in a primary care setting
