Job Description

Start Date: Ideally June 2025, flexible for the right candidate

Main duties of the job

We are currently looking for an enthusiastic, highly motivated, proactive, and forward-thinking GP, committed to providing high standards of holistic clinical care and professional conduct to our patient population.

The right candidate needs to be able to work effectively within a collaborative and integrated MDT environment and will contribute to the daily patient care, including acute and chronic disease management, with a view to promote care planning and proactive patient management initiatives.

There is scope to work a hybrid role combining remote and face-to-face working.

Rated 'Good' by the CQC, we are committed to providing high-quality, patient-centred care through our multidisciplinary team. As part of the Hounslow health Primary Care Network, we collaborate closely with partner practices to deliver innovative and integrated healthcare.

We currently serve a list of over 14,000 patients with a team that includes GPs, nurses, clinical pharmacists, healthcare assistants, physician associates, social prescribers, mental health practitioners, and physiotherapists. We offer a very well-supported and inclusive working environment, underpinned by a progressive care navigation system. We have regular MDT and clinical meetings and a strong ethos of continuous learning and improvement.

Job responsibilities

Person Specification Qualifications
  • Must be a qualified GP registered with the GMC
  • UK Registration
  • Applicants must have current UK professional registration
  • Experience of working in General Practice
  • Disclosure and Barring Service Check
  • This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
  • Experience using SystmOne clinical system
  • Interest in teaching, training, or specialist clinical areas (e.g., women's health, mental health, frailty)
  • Willingness to take on a lead role or contribute to PCN and service development work
