Remote CHC Nurse Assessor - Caseload & Care Coordination

Medacs Healthcare is recruiting a CHC Nurse Assessor (Band 6) for a fully remote locum role in the UK. The post involves 37.5 hours per week, Monday-Friday, assessing care needs and coordinating packages while liaising with NHS services and local authorities.

The ideal candidate will have CHC assessment experience, current NMC registration (RGN/RMN/RNLD), and the ability to manage a caseload with strong clinical decision-making and communication skills.
